Continuity mistake: When Andy is lying on the ground unconscious The Wendigo rips open his shirt popping all the buttons, but in the closeup of Andy's face his shirt is still buttoned up. (00:38:50)

Visible crew/equipment: When Phoebe joins her sisters in the kitchen before her first day at work, the reflection of the moving boom mic is visible on the glass cabinet, at the top of the screen. (00:10:05)
Which Prue Is It, Anyway? - S1-E16
Continuity mistake: While Phoebe explains the slam man training system Piper puts on Phoebe's boxing gloves, and when Prue says that demons don't play fair Piper playfully raises her left arm to Prue's forehead, but in the next shot facing Prue it's Piper's right arm that's raised. (00:04:00)
Continuity mistake: While Andy and Darryl are at the station talking about why Andy's been "walking around...like a zombie," Andy places a pencil behind his ear, but when it cuts to Inspector Blakely walking in behind Andy the pencil has vanished between shots, but it reappears again in the next shot. Then when Andy borrows the photograph of the knife, once again the pencil vanishes between the closeup and wideshot. (00:09:10)
Chick Flick - S2-E18
Character mistake: After the horror movie characters were killed in the attic, the sisters hear the door bell ringing. When they get to the bottom of the stairs and start walking towards the front door, you can see the actor playing Billy smiling even though they're all supposed to be on edge.
Revealing mistake: The sound of Phoebe writing with a permanent marker was added in, as you can see that the marker never touches the paper. Then after a few seconds Phoebe stops writing to listen to Prue and her marker isn't moving but the sound of writing can be heard. (00:05:30)
Plot hole: When Yen lo enters the manor in the attic. Where did the water on the floor he comes through come from? Windows were closed, no visible leak, and the sister took precautions to make sure there was no water in the house. Also after he attacks and gets the dragon blade, he exits the house through the sink filled with water. Piper had already said to make sure there was no surfaces of water in the house and had Phoebe and Paige shut the water main off.
Suggested correction: There is a previous scene that shows a visible leak in the windows and an obvious puddle of water on the floor of the attic. This is the same puddle they later enter limbo through. As for the sink, Phoebe (in Paige's body) explains that she had been soaking potion pots and forgot to drain the sink. Turning off the valve wouldn't dissipate water that is already in the sink.
Factual error: Phoebe is going to be burned at the stake for witchcraft like they're starting the Salem witch trials again, but witches weren't burned, they were hanged.
Plot hole: When the man and woman receive Piper and Phoebe's powers, the lady freezes the man and Phoebe due to fear, but good witches don't freeze, so if Phoebe froze (which she shouldn't have due to only her active powers lost) then why did the man? If he had Phoebe's powers then he isn't mortal and therefore should not freeze.
Character mistake: When Miles is shot, the cops apparently don't think it's necessary to tend to him to see if he's okay.
We're Off to See the Wizard - S4-E19
Other mistake: Leo tells Phoebe that fire throwing is an upper level demon power, but we know Phoebe had that power in her past life, Piper even reminds her of this earlier in the episode.
Pre-Witched - S3-E17
Revealing mistake: At the beginning of the episode Piper and Leo are in the shower together - at the bottom of the screen you can see Piper has on a flesh colored towel.
Sleuthing With the Enemy - S3-E8
Other mistake: Cole is supposed to be one of the most powerful demons alive, but a small cut from Piper weakens him to the point where he can't even shimmer or keep up his "Cole" appearance.
Love Hurts - S1-E21
Continuity mistake: When Prue shoots the Darklighter arrow at Alec, he orbs out and the arrow sticks into the wall behind him. In the next shot, the arrow has disappeared. (00:25:45)
Continuity mistake: At Phoebe's high school reunion, the gym teacher holds out a name tag in her left hand, but then Paige takes it from her right. (00:07:50)
Witchstock - S6-E11
Continuity mistake: When Chris orbs Phoebe from Hong Kong, Phoebe raises her arms to hug Piper twice. (00:00:50)
Continuity mistake: When Anling unmasks Yenlo, Yenlo's left hand is covering his wound. In the next shot, his hand is down by his side. (00:03:50)
When Bad Warlocks Go Good - S1-E18
Continuity mistake: When Brendon is about to sacrifice the young woman he has the tip of the dagger touching her chest in one shot and in the next shot, when Prue breaks open the door, it is about six inches above her. Also, when the victim is talking to Andy she says that she felt the tip of the knife, that he was about to drive it into her, but he was stopped by three woman. (00:24:15)
Factual error: Numerous times in the establishing shots of San Francisco, the Embarcadero Freeway is visible. While the bridge did run along the San Francisco Bay, it was demolished in 1991 - seven years before the series even started.
