Revealing: At the end of the opening credits, you can see Angel's reflection in the puddle on the floor. Since he is a vampire, this should not happen.

starring Alexis Denisof, Amy Acker, Andy Hallet, Charisma Carpenter, David Boreanaz, J. August Richards, James Marsters, Mercedes McNab, Stephanie Romanov, Vincent Kartheiser (add more)
Across whole show
Character mistake: At the Bronze in the Buffy episode "Angel", Angel says that he hasn't fed on a human being since he was cursed. But this is actually untrue: in "Why We Fight", flashbacks show he fed on a Navy submarine's dying commander so he could sire him in order to save a captured Nazi submarine, and in "Orpheus", a flashback/memory shows that he also fed on a gunshot victim in a cafe in the 70s.
All of series 1 (season 1)
Character mistake: The slogan of Angel Investigations changes several times. In some episodes, it's "We help the helpless," while in others it's "We help the hopeless."
City Of (season 1, episode 1)
Revealing: In the last scene of the episode, Angel kicks the guy out of the glass building. This guy's a vampire, and you can see his reflection in the glass windows. Vampires aren't supposed to have reflections.
Continuity: When Angel kicks the vampire out the window, he does it with little effort and movement, but when the shot goes from outside the building, he's swinging his body like he's put more into it.
Lonely Hearts (season 1, episode 2)
Revealing: In the scene when Angel, a vampire, runs after the barman towards the end, after the barman has tried to pass on the demon to Kate and Angel stopped him, there is a mirror, and when Angel runs past it, he has a reflection.
Continuity: In the scene where Detective Lockely arrests Angel in the apartment, the window behind them shows that it's daytime. But when Angel jumps out the window, it's night time.
I Will Remember You (season 1, episode 8)
Continuity: In the season one episode 'I Will Remember You' when Buffy and Angel are talking in the sewer Buffy's hair goes from being just a few strands over her shoulder to having a whole side over it, and back again.
Expecting (season 1, episode 12)
Revealing: When Angel stands at the cabinet with a file folder in his hand, you don't see the folder in the reflection in the window (which Cordelia uses as mirror to put on her lipstick) but you can see the cabinet.
Visible crew/equipment: In the scene where Wesley and Cordelia are in the doctor's office , the boom mike appears at the top of the frame. (Widescreen Edition).
Continuity: When Cordelia is putting on lipstick she uses the windows as mirror. In the reflection we see the top drawer of the cabinet is open the whole time, but it is later opened by Angel.
The Prodigal (season 1, episode 15)
Revealing: In the scene where Kate finds her father dead, just after the shot of his neck wounds, she is sitting over him and you can see the father blinking a few times.
Visible crew/equipment: In the scene where Angel is talking to Kate about the "evil thing" , the boom mike appears at the top of the frame. (Widescreen Edition).
Deliberate "mistake": In the flashback to Angelus rising from his grave, his and Darla's breath is visible as they exhale. This was going to be removed in post-production, but left in due to budget restrictions.
The Ring (season 1, episode 16)
Factual error: According to the show, Angel learned quite a few languages when he was evil. However, when he tries to start a conversation with a red-faced demon, he makes terrible mistakes. He said "Hablo Espanol?" which literally means "Do I speak Spanish?" and he said something like "Are you from Byelorussia?" in a tongue that hardly resembled Russian, although according to the captions, it was said in Russian.
Continuity: In the final fight, Angel gets stabbed on his right side. In the slow-motion bit before his opponent chooses not to fight any more, the wound switches to his left for one shot.
Five By Five (season 1, episode 18)
Continuity: When Faith is sitting at the window and looks at the piece of broken glass she used to torture Wesley, it has a different shape than the one she first takes to torture Wesley. When she drops it out the window it has its original shape again.
Sanctuary (season 1, episode 19)
Revealing: When the insect creature is crawling on the ceiling its hair doesn't falls down towards the floor, it "falls" towards the ceiling.
War Zone (season 1, episode 20)
Revealing: When Alonna is kidnapped, there is a vampire on the left side of the screen who is not wearing any mask over his face, though sunlight is falling on him.
Revealing: At the vampires' nest, when Knox is talking about Gunn's team, the vampire wearing the green sweatshirt and leather vest doesn't have yellow eyes like the other vampires do. And the female vampire doesn't either later, when the two groups face off after Gunn's sister is sired.
Blind Date (season 1, episode 21)
Character mistake: Angel comments that Vanessa was acquitted because of a hung jury. A hung jury means the jury couldn't come to a unanimous decision; the case would require a new trial, while an acquittal means the jury found her not guilty, and she couldn't be tried again for the crime.
To Shanshu In L.A. (season 1, episode 22)
Continuity: In Season One Finale, "To Shanshu in LA" when Wes is lecturing Cordy on wants and desires, the number of bites taken from the doughnut changes, in one instance it breaks in half, most visibly when Wes takes a bite from Cordy's doughnut on the angle change.
Revealing: In "To Shanshu in LA" in season one, as Angel is pulling out of parking lot to follow the W&H Trucks, you can see his reflection in his rear view mirror (vampires have no reflections). Also odd that throughout the first season the rear view mirror is removed in some episodes.
Continuity: In the season one finale "To Shanshu in LA," near the end Angel is fighting Vocah in the mausoleum and Angel rips off Vocah's mask, revealing the maggoty face underneath. In subsequent shots of Angel from behind Vocah, you can still see the edges of the mask on his face.
Continuity: When Wesley is explaining that Angel feels like he doesn't belong in the world, the amount of donut he's holding changes.
Visible crew/equipment: At the end of the episode where Wesley is studying the Shanshu scroll in Cordillia's apartment, the boom mike appears at the top of the frame TWICE.
Judgment (season 2, episode 1)
Revealing: You see Cordelia and Wesley walking without Angel through the gym in the mirror (because vampires have no reflection). In the Widescreen Edition you can see Angel waiting at the right side of the screen to join in a "normal" shot.
Revealing: When Angel and co. are at the gym, Angel's reflection is visible in the mirror.
Visible crew/equipment: When Angel is fighting the demon who protects the pregnant woman, at one point a crew member (in ordinary street clothes) shows behind them for a few frames.
Visible crew/equipment: When Angel gets on the horse to joust, you can see a part of a camera at the right side of the screen. (Widescreen Edition)
Revealing: Just after Angel kills the quan-eyed demon, he exits the room behind the smashed mirror. On the widescreen DVD you see Angel's reflection in the mirror to the right of the smashed mirror (even though vampires don't have a reflection).
Are You Now Or Have You Ever Been (season 2, episode 2)
Continuity: When Angel hides the bag of money up on the shelf in the basement, he is not wearing glasses. However, in the close up shot of his arms and the bag though, he is.
Untouched (season 2, episode 4)
Continuity: When Cordelia is bandaging Angel in the hotel lobby, she puts two pieces of tape on him: one at the base of the gauze and one over the wound itself. When he takes his shirt off in his room, he only has the one at the base.
Guise Will Be Guise (season 2, episode 6)
Character mistake: In this episode, Gunn wonders how he could have lived in Los Angeles all his life without knowing about all the "weird ass stuff" going on. But flashbacks in "Double or Nothing" show that he knows demons exist there.
Darla (season 2, episode 7)
Continuity: When Angel is talking to Darla on the phone there is a shot of Cordy, Wes & Gunn and you can see Cordy unfold her arms but when it goes back to Angel you can see her arms folded again.
The Shroud Of Rahmon (season 2, episode 8)
Other: In the episode "The shroud of Rahmon," when Angel changes into his vampire face, his teeth don't change into fangs.
The Trial (season 2, episode 9)
Revealing: In the scene where Angel fights the demon for the first of his trials, upon cutting the demon in half for the second time he chains the two halves to the walls. When he drags the upper half of the demon to the wall, the green legs of the actor are momentarily visible, when they should have been removed by CGI.
Reunion (season 2, episode 10)
Revealing: When Angel kicks in Lindsay's door, it opens cleanly into the room. The doorknob latch in a real door would have torn out part of the frame if it was not turned back with the doorknob first.
Revealing: When Angel spins his car around after Cordelia's vision, there are skid marks on the road from previous takes.
Blood Money (season 2, episode 12)
Continuity: In season three episode "Anne" of BTVS, The character "Anne" who later comes to season two of Angel running a youth shelter had a rather large red tattoo of half a heart on her arm. However, no trace of it is to be found when she is on Angel. Tattoo removal is a rather pricey procedure, and she seems to be tight on cash until she gets the two million. Even if W&H would have paid for the procedure, there would be some scarring or residual pigments for such a large tattoo. (It's definitely the same character - Anne says that she used to think vampires were cool, until she met one. That's a reference to her first appearance (as "Chanterelle"), in which she belonged to a vampire-loving cult.)
Happy Anniversary (season 2, episode 13)
Revealing: When Wesley accidentally turns off the power, the room briefly goes dark, then lights up with moonlight/streetlight from outside. In the real world, that light would have been visible the whole time in the background.
Epiphany (season 2, episode 16)
Deliberate "mistake": In the scene when Angel and Wesley are driving down the road, you can see that the big windshield, which is normally at the front of the car, has been removed. So that there would be no reflections of the crew that were filming those scene(s)
Disharmony (season 2, episode 17)
Continuity: When Harmony enters Cordelias bedroom, the strap on Corelia's shirt falls off her shoulder and then magically is back in place after the angle change.
Continuity: When Cordelia and Harmony are on the couch talking and drinking, the towel on Cordelia's head goes from on her shoulder to behind in the change from wide to close up.
Continuity: In 'Disharmony' Willow's hair is over her eye when she says "Thanks for the affirmation", but when we see her again a second later its tucked behind her ear. The shot wasn't on Cordelia long enough for her to have tucked it back.
Other: When Harmony spills her pig's blood on the keyboard, the computer shorts out, and the monitor flickers off, but the mouse cursor is still visible.
Other: At the very end, as Angel walks out the door he reflects in the panes of glass on the door.
Through The Looking Glass (season 2, episode 21)
Continuity: when Angel transforms into his inner beast for the first time he is seen ripping off the right leg of an Imperial guard, but in following shot the leg is intact and it stays like that for the rest of the fight.
All of series 3 (season 3)
Factual error: Connor has been raised since he was a few months old by Holtz, a man with a British accent, in a dimension where there are no other humans. There is no possible way he could have an American accent, yet from the moment of his return he speaks with one.
Heartthrob (season 3, episode 1)
Continuity: In the scene where Angel has just returned and given Cordelia her necklace, watch Cordelia's hands; they keep changing position in each shot. In one shot she is trying to put the necklace on, in the next shot she appears to have given up and brings her hands down and bundles the necklace in her hand. Then when the shot changes to behind her, she is trying to fasten the necklace behind her neck again. This happens a few times during this scene.
Character mistake: In the first flashback to Angelus and James in Marseille, Angelus briefly speaks with his modern-day American accent before reverting to his Irish accent.
Revealing: When James, a vampire, chases Angel and Cordelia into the subway, James' reflection is visible in the windows of a passing train.
That Old Gang Of Mine (season 3, episode 3)
Revealing: Supposedly the demon Merl has no tongue, but when he is telling Angel the apology he wants, it can be seen.
Offspring (season 3, episode 7)
Continuity: While waiting for Holtz to awake, Sahjhan is able to hold a bowl of mystical powder, and smoke a cigarette. But subsequent episodes establish that, on Earth, Sahjahn is incorporeal, and thus he shouldn't be able to perform those actions.
Quickening (season 3, episode 8)
Continuity: Sahjhan establishes that he is incorporeal on Earth, but when he takes Holtz to meet his demon minions, he manages to knock on the door.
Dad (season 3, episode 10)
Continuity: When Lorne covertly warns Angel that the hotel is bugged, he places a note in Angel's pocket with his hand on Angel's shoulder. When Wolfram and Hart watch the video feed later, the hand that was on Angel's shoulder is now in front of Angel's chest.
Couplet (season 3, episode 14)
Continuity: When the demon Angel and Groo are chasing crashes through a wall, the resulting hole lets in enough sunlight to force Angel into the shadows. A few shots later, Wesley notices Angel standing in the entrance to the hole. Only a few moments have passed, so Angel shouldn't be able to stand where he is.
Sleep Tight (season 3, episode 16)
Revealing: When Angel is seen gulping the blood from the cup (right before Lorne tells Angel that Connor needs some papa-love) you can see this is a special-effects cup (it's like one of those freezer mugs, where there's a layer of liquid on the outside). If Angel were really drinking out of the cup the way it's shown, he would have blood all over his lips and it would be running down his chin.
Forgiving (season 3, episode 17)
Factual error: Wesley has his throat cut, deeply enough to at least nick the artery (based on the amount of blood) and the windpipe (based on his difficulties breathing and talking later on), yet he somehow survives being left alone in the park for over 24 hours. In reality, he would have been dead long before then.
A New World (season 3, episode 20)
Revealing: The Axe that Groo throws at Conner at the start of the episode (Obviously CGI) doesn't match the movements of his hand, it leaves his hand before he actually throws it.
Revealing: The first stake that Conner fires from his stake cannon (Obviously CGI) doesn't seem to emerge properly from the barrel of the cannon, it emerges out of thin air from the side.
Tomorrow (season 3, episode 22)
Continuity: The entire area around Cordelia and Skip is supposed to be frozen in time. However, there is a car driving freely across a bridge in the background behind Cordelia. (The background is blurred, so it is difficult to see.)
Ground State (season 4, episode 2)
Revealing: In 'Ground State' when Gwen is walking towards the lift in Irwin's building you can see Angel's reflection on the shiny door. Vampires don't have reflections, as is demonstrated at various other times in this show.
Factual error: In the flashback to Gwen as a child in 1985, she is dressed in a jacket tagged with an "Old Navy" zipper. That clothing store chain wasn't founded until 1994.
The House Always Wins (season 4, episode 3)
Revealing: When Gunn, Fred and Lorne run out to the Las Vegas strip, a large crowd of bystanders can be see seen in the background watching the show being filmed (as said in the DVD commentary).
Spin The Bottle (season 4, episode 6)
Continuity: When Angel is talking to Cordelia in the garden at the very end of the episode "Supersymmetry" he has a cut on his cheek. The beginning of "Spin The Bottle" is the continuation of that scene, but Angel's face no longer has a cut.
Apocalypse, Nowish (season 4, episode 7)
Continuity: After Cordelia kisses Connor, her hand is on his cheek. Just before she says "and if this is the end", in the shot of Connor she removes her hand, then the angle changes to show her, and her hand's suddenly back on his cheek again.
Awakening (season 4, episode 10)
Other: During Angel's fight with the Beast, he is choked by the throat and begins gagging. Since Angel doesn't breathe, there's no air for him to lose, so he shouldn't gag.
Audio problem: When the beast bursts into the office, there is a close up of Cordelia, her voice is heard to yell Angel but her mouth hardly moves.
Soulless (season 4, episode 11)
Continuity: When Cordelia looks at the monitor, the reflection of Angelus's cage shows Angelus standing at the bars next to Connor. It cuts to a direct shot, and Angelus is sitting on the ground on the other side of the cage.
Calvary (season 4, episode 12)
Audio problem: During Lilah's failed attempt to rescue Angelus, when Gunn shoots at Angelus with the tranquilizer gun, Angelus passes out then hits the ground. When he is about to pass out, we hear the sound effect of him hitting the ground before he even begins to fall over, and instantly in the next camera angle he hits the ground.
Salvage (season 4, episode 13)
Continuity: In the scene where Angelus is drinking from dead Lilah, in one shot Lilah's hand is hanging by her side while Angelus holds her, but in the next shot - just before he drops her - Lilah's hand is gripping for support at Angelus shoulder.
Continuity: When Wesley tells Faith that Angelus is back, she says, "Step away from the glass" and hangs up the phone. When Wesley jumps from his chair (to allow Faith to smash through the glass and escape) you can see Faith behind the glass with her hands up to her face and standing, but in the close up of Faith her hands are at her sides and she is only just getting out of her chair.
Continuity: When Cordelia reveals to Connor that she's pregnant, she places her hand on his face. In the very next shot, her hand is gone.
Orpheus (season 4, episode 15)
Continuity: When Cordy throws the knife at the door after Willow leaves, the knife hits at an impossible angle from where it was supposed to have been thrown.
The Magic Bullet (season 4, episode 19)
Continuity: When Fred is running from the oncoming white car, she runs toward a blue car getting fixed on the side of the road, but jumps out of the way as the white car hits the blue car. Fred hits the ground and rolls over to the fence, but in a following shot it shows Fred only just hitting the ground and she is on the grass, not next to the fence.
Continuity: When Angel and the others infect Connor with Cordy's blood, Gunn rips open Connor's shirt. In the following shots, the shirt closes and then opens again.
Peace Out (season 4, episode 21)
Continuity: When Jasmine picks up the car and throws it at Angel, it cuts to a shot of Angel moving out of the way then shows the car falling from the bridge. First mistake, Jasmine disappears from the shot when the car is falling, and second, the car falls directly straight and at a higher point than Jasmine actually threw it.
Home (season 4, episode 22)
Audio problem: The lips of the Newscaster's who reports on Conner taking over the sports store do not match what she is saying at all.
All of series 5 (season 5)
Continuity: In the season 5 episode 'Destiny' we see that the first time Angelus meets William, he has already adopted a more cockney accent, but in an earlier episode of BtVS (Fool For Love) Angelus asks Spike "Since when do you talk like that", suggesting he changed his accent after they met.
Conviction (1) (season 5, episode 1)
Continuity: The girl being rescued at the beginning goes from standing close to the wall, looking blank in one shot, to being further away from it looking concerned in the next.
The Cautionary Tale Of Numero Cinco (season 5, episode 6)
Continuity: When Angel is outside looking for Number 5, he is stabbed in the stomach by a demon. The next scene is when Angel returns to Wolfram & Hart right after this happens and he is still wearing the same clothes. He mentions he is still in pain and yet there are no rips or tears in his shirt and no blood stains.
Revealing: When Angel says to Wesley that "Fred has got something," and then walks out of the room, Angel's reflection is visible in a window pane in front of him. It's very brief, but it's there.
Factual error: When the mysterious person hands Cinco the business card during the 50's flashback (in the restaurant where he is sitting waiting for business), it says "Wolfram & Hart, Attorney's at Law." Clearly, there should be no apostrophe in "Attorneys" in this context - not a mistake a professional law firm would make.
Destiny (season 5, episode 8)
Plot hole: After learning about the prophecy, the gang realise that Spike's headed off already, getting a head start. Angel grabs another car and heads after him, but why use a car? We find out in the first or second episode this series that he's got access to a helicopter, which he uses to get a long way ahead of some bad guys, and which would have saved a lot of hassle this time. Can't imagine Wolfram and Hart's pilots only work in the daytime.
Damage (season 5, episode 11)
Continuity: When Angel is talking to Spike after he's had his hands re-attached, Spike is hooked up to an IV. If you watch the IV bag between shots, the fluid level changes from 1/4 full to 1/2 full.
You're Welcome (season 5, episode 12)
Character mistake: Near the end of the episode Angel says "All I did was beat up a tiny Texan" referring to Lindsey. But at the end of season one Lindsey told Angel he was from Oklahoma.
Why We Fight (season 5, episode 13)
Plot hole: In the flashbacks, there is hardly any use of language from the 1940s.
Smile Time (season 5, episode 14)
Deliberate "mistake": When Fred, Wesley, Gunn and Angel plan to go to the studio where "Smile Time" is filmed and they are about to leave, Puppet Angel takes a sword down from the wall behind his desk. Fred opens the door and Puppet Angel is still behind his desk. However, as they walk down the hall when the dramatic music begins to play, Puppet Angel is suddenly in front of the group. There wasn't enough time for Angel to get in front of them when Fred opens the door.
A Hole in the World (season 5, episode 15)
Continuity: When Gunn is talking to Wesley near the start of the episode, he mentions that he's tracked Lindsay down, and that he's been living under the name "Doyle", and holds out the lease of his apartment. There's a close-up of the lease a few seconds later, in which the name "Lindsay McDonald" can be seen.
The Girl in Question (season 5, episode 20)
Revealing: During the whole slow motion fighting scene in the bar the punches clearly miss their targets.
