Mistake Continuity: When Seagal and McGillis are driving through the field he is shown driving from both sides of the vehicle. One moment on the left and the next very quick shot on the right.

Mistake Plot hole: The Steven Segal character is in a coma for a number of years, how is it that on the day he awakes from the coma, he not only has the strength to move his bed down corridors but also knows the layout of the hospital?

Mistake Continuity: The hit man stands in front of a bank of elevators in the hospital. He turns to run to the other bank and a hand reaches across to shut the elevator door as he approaches.

Mistake Continuity: Steven Seagal and Kelly LeBrock are in a vehicle and are being chased through some fields by the cops. There are several bullets shot into the windshield of their car. During the course of the chase it appears that the formation of the bullet holes changes throughout the chase.

Mistake Continuity: In the scene where Segal and LeBrock are being chased across the field, and they drive the jeep through a gazebo, Segal is driving. However, for a split second as the jeep passes through the gazebo it looks like Segal is in the passenger seat and a stunt driver is at the wheel.

Mistake Continuity: This is during the car chase scene when Steven Seagal and Kelly LeBrock are in the jeep. Throughout much of this scene, the license plate on the front of the jeep is on the driver's side. But, towards the end of the sequence, there are a couple of seconds where the jeep shows the plate to be on the passenger side. It then goes back to being on the driver's side as the chase ends.

Mistake Continuity: In the train station scene, when O'Malley is shot, watch the front of his shirt as he hides behind the cab so he can start shooting the bad guys. He was running away from them when he gets hit once in the back. Then, as he turns around to fire back behind the car, there are two bullet holes in his shirt with blood dripping down like he had been hit. But he was only shot once in the back.

Mistake Deliberate "mistake": Steven Seagal is in a coma for 6 years. All his hair grows long, and he has a long beard. Problem is, the beard is a goatee style, and there is no hair connecting the beard to his hair. He is seen to be clean-shaven in this area, which means someone at the hospital has taken the time to shave that area over the course of 6 years, but they let the rest of his hair grow long?

Mistake Continuity: Steven Seagal is wearing a suit and vest in the beginning of the film when he is all shot up. He is then wearing the same suit and vest later in the film when he recovers.

Mistake Visible crew/equipment: In the shoot out scene at the train station between Captain O'Malley and the bad guys, through O'Malley's shirt you can see the patch strip they use to make the fake bullet holes when he gets shot. You see it well because he gets shot in slow motion.

Mistake Continuity: In the scene where Seagal and Lebrock first jump into the jeep to escape the bad guys at the House, the roof of the jeep gets shot up by a gunman above them. Seconds later, when they crash through the fence, the roof of the jeep is intact.
