Mistakes
In the scene where Neo unloads two 9mm Berettas at an agent who dodges all the shots, right after he runs out of ammo, the camera is facing Neo dead on and the recoil slides on the pistols are closed. The next shot where he drops them and says, "Trinity... help." the slides are locked back with the ejection port open. See more...

When Morpheus and Neo are in the elevator to see The Oracle, look and the wall to the left of Neo's head when they focus on him. Notice the name "Kym" scratched in? Kym Barrett is the name of the costume designer. See more...
The Matrix
Continuity: In the scene with Laurence Fishburne fighting the agent in the bathroom of the building they were trapped in, the agent punches through a brick wall, yet next time we see the wall, there is no damage.
