Jaws 4 (1987)

Revealing mistake: In the scene where the shark has Michael cornered in the old, rusted boat; as he's about to use his oxygen tank to rocket up to the surface, the shark breaks through the wall and when it does, a large number of thick tubes can be seen running into the shark to make it work.

Plot hole: When Jesse falls through the casket when he's trying to escape the mausoleum, he knocks a corpse to the ground with him. Wouldn't it stand to reason that this corpse would be reanimated when all the others are?

Visible crew/equipment: When Curt and Julie move to get a better view of the re-animation process, at the bottom of the screen, a boom mic is visible.

Revealing mistake: When Bill and Ted are falling down to Hell, and they're playing 20 Questions, when Ted goes to give Bill a high-five, the plank Ted's laying on can be seen when his shirt stretches over it as he high-fives Bill.

Re-Animator (1985)

Continuity mistake: When Cain and West are in the morgue trying to re-animate the cadaver, a phone rings outside at the security desk and when we see it, the buttons are facing outward, but when Megan goes down to the security desk, we see the phone is now turned completely around.

Revealing mistake: In Darkman's daydream where he launches himself across the boardroom table and chokes out Durant, look at his mouth when Durant rips his mask off. You can see Arnold Vosloo's real teeth behind the fake ones.

Critters 2 (1988)

Revealing mistake: Nearing the end of the film when the giant critter ball is roll back to town and the good guys are following in the truck, you can see the shadow of the truck in front of the critter ball pulling it along. It's visible in more than one shot.

Visible crew/equipment: Toward the end of the film when the critter ball enters town and the truck swerves to the left of the ball, you can see the feet of a crew member running behind the ball, pushing it along the street.

Psycho III (1986)

Audio problem: In the scene after Norman goes for a late-night swim, when he's telling Maureen that he slipped in the mud, you can see that his mouth never moves, yet he speaks the words "I slipped in the mud."

Visible crew/equipment: At the very end of the scene where McClane and Zeus take the businessman's car, just before the scene cuts, the boom mic can be seen reflected in the yellow car's back window as it is lowered, waiting for the "cut."

Deliberate mistake: In the whole of the 'Psycho' series, there has never been a closet door next to the dress cabinet in Mother's room, but for this movie there is suddenly a closet.

Continuity mistake: When Murphy, Connor, and Romeo arrive at Doc's bar the first time, a cigarette can be seen tucked behind Murphy's left ear, but when the boys enter the bar, it is now behind his right ear.

Other mistake: When Special Agent Bloom arrives at the beginning of the film, there's a shot of Greenly spraying breath spray into his mouth and while he does this, he's bent over, but in the very next shot, he is standing completely upright.

Revealing mistake: In the scene just after Miguel is bit and Sarah is amputating his arm, when it cuts to a relatively close shot of her working the machete, you can see the fake blood spurting out from underneath her right hand.

Visible crew/equipment: In the beginning when Johnny gets out of the car and we have a bit of a wide shot, before he closes the car door, you can see the top of the camera reflected in the window.

Continuity mistake: In the marketplace standoff between Indy and his would-be attacker, in shots from behind Indy, he's facing one way, but in frontwards shots, he's facing a different way and when it cuts back to behind him, he's facing the other way again.
