The Wedsworth-Townsend Act - S1-E1

Visible crew/equipment: As the camera pans to the left at the start of this episode, while everyone is asleep in the dorm upstairs, a crew member standing near the production's lighting equipment is visible between Squad 10 and the wall. Also note that the lighting equipment is gone when the guys are dispatched.

An Ounce of Prevention - S6-E13

Visible crew/equipment: When Roy and Johnny are on Tom Jensen's show, the outline of Johnny's lavalier mic transmitter is visible on the front, right of Johnny's shirt, over his belt, as he and Roy begin their demonstration, and also after Jensen's crew member electrocutes himself when Johnny and Roy carry and lay him down near the table. This has nothing to do with the wired mics that are clipped onto Roy's and Johnny's ties for Tom Jensen's show.

An Ounce of Prevention - S6-E13

Continuity mistake: At the ferris wheel, when the boy starts to climb down to help the man in the lower seat, in the closeup he steps on the two yellow, fluorescent tube light bulbs causing the bulbs to plummet and shatter on the ground below. The problem is that those two fluorescent bulbs did not even exist in the prior wide shots, that lamp socket space was empty.

Insanity Epidemic - S6-E14

Visible crew/equipment: At the skating rink, when the guys start walking on the ice and go to the two clowns, the outlines of the mic transmitters and wires are visible on Johnny's shirt, as well as on the back of the blonde skater's green outfit. Then, when the ambulance attendants are walking on the ice, the outline of the lead attendant's mic transmitter is visible on the front of his shirt.

Insanity Epidemic - S6-E14

Visible crew/equipment: At the gas station with the broken pump nozzle, when Roy and Johnny are tending to the woman with the cut on her head, the outlines of Roy's mic transmitter and wire are visible on the left side of his shirt, above his belt. Additionally, note that the office's left pane of glass has been removed to prevent the reflection of the camera facing it.

Surprise - S4-E6

Visible crew/equipment: At the scene of the swaying sign accident, when Roy and Johnny get the gear out of the squad the outlines of the mic transmitters are visible on the left side of Johnny's shirt and right side of Roy's. Once they're on the roof both of the mic transmitters' outlines are on the right sides of their shirts, and later when Roy and Gil are pulled up onto the roof, the bulging outline is visible again at Roy's right side.

Paper Work - S6-E11

Continuity mistake: When Johnny and Roy are with the boy who is stuck in the storm drain, note that the shoes both paramedics are wearing are flat bottom, dark grey, water-type shoes, and not their regular black uniform shoes. Then, when the boy is lying on the grass, Johnny's in his uniform shoes, though Roy is still in those water-type shoes.

Paper Work - S6-E11

Continuity mistake: When the car swerves to miss the dog and hits the fire hydrant, the area of ground around the front tire is bare, but in the closeup as the man and his son exit the car, that area is now covered in vegetation, enough to cover half the tire.

Paper Work - S6-E11

Continuity mistake: When the new supply nurse gives Roy the box of supplies, in his right hand Roy holds the receipt and supply box, and in his left hand is the handie-talkie, but in the closeup Roy holds the supply box with his left hand and a bag of saline in his right hand.

Paper Work - S6-E11

Other mistake: When the little boy is stuck in the storm drain with the rushing water from the broken hydrant, Johnny climbs into the manhole and is submerged up to his shoulders in water while dealing with the boy, but when they lift the boy out and lay him on the grass, Johnny is quite dry except for some wet patches on his shirt.

The Steel Inferno - S7-E1

Continuity mistake: When Charlie asks Dave how Sue is, Charlie has an oxygen mask on with the elastic around his head, but after Dave tells Charlie that Sue's fine and that Johnny's got her on O2, in the wideshot as Dave walks away the oxygen mask has vanished from around his head.

Show generally

Other mistake: In the interior shots of Station 51 (the studio set), when facing the rear apparatus bay door, the right wall has a door to Captain Stanley's office, a very large map, the radio receiver base station where they respond to dispatch, the kitchen doorway, and a double-door closet, that's it. However, when Station 51 is shown in the exterior shots (the real Station 127 aka 51), as either the engine or squad exits when the apparatus bay door is up, we see that the right wall has many more doors on that wall. Additionally, the opposite wall on the set has only two doors - one to locker room/bathroom, and one to the dorm, however at the real 127's, that wall has three doors - two of them are to the dorm.

The Steel Inferno - S7-E1

Visible crew/equipment: When Evelyn and her husband pull up in front of Graymar Plaza, while they're talking in the car, in the shots facing Evelyn the reflection of crew members standing beside the car, on the driver's side, is visible on the building's glass doors.

The Steel Inferno - S7-E1

Other mistake: When dispatch drops tones for the fire at Graymar Plaza, we see Engine 8 rolling out of the apparatus bay and heading down the street, but the problem here is that this footage of Engine 8 is from season 5, episode "Equipment", when Johnny was at Station 8 doing overtime, and we can actually see Johnny sitting there on Engine 8 as it rolls by.

The Old Engine - S3-E2

Visible crew/equipment: When Engine 51 gets dispatched to their first run on the new Ward LaFrance engine, Captain Stanley tells Johnny and Roy to get their gear to go with them. When the paramedics go to the closet to get the turnout coats, the bulging outlines of the mic transmitters and wires are visible on the sides of their shirts before putting on the turnouts. The outline of Johnny's mic transmitter is also visible when he first walks over to the new engine.

Trivia: The producers had Kevin Tighe and Randolph Mantooth sit in on paramedic classes (with students who were to be future paramedics), and they also went on extensive ride-a-longs with the Los Angeles County Fire Dept, in order for the two actors to have a realistic appearance, and give a believable performance during the scenes. However, the two actors did not take the written exams. They have never been real certified paramedics.
