Revealing mistake: After Roy and Johnny get to the hospital with the 18 month old baby who stopped breathing, when Johnny is rushing up the hallway giving mouth-to-mouth to the baby, we can see that the happy little baby is wide awake and even smiling.

Visible crew/equipment: When Roy and Johnny get back to the station talking about the ten fire inspections and two runs, the moving shadow of the boom mic is visible on the cabinets above Chet.

Other mistake: When the patient with the heart wound is brought to the ER, the anesthesiologist brings the cart with a ventilating unit close to patient, then there are two closeups of Bird's Mark 4 unit and the bellows housing showing inflation and deflation, and on the bellows unit it reads "Animal Anesthesia Ventilator" made by Bird. This is also the same closeup we see of the ventilator unit in 4x18, "Prestidigitation," during the surgery on Brackett's father.

Visible crew/equipment: During the fire at the chemical plant, a couple of shots after we see Captain Stanley walking between Roy and Chet as they're manning two hoses on the roof, we see a shot of Engine 15 as it heads toward the camera and then drives past it, and the reflection of array lighting is visible on the engine's surface.

Visible crew/equipment: Just as Roy brings the victim with the heart wound into treatment 2, in the upper left corner we can see the moving shadow of the boom mic.

Other mistake: When Johnny and Roy are in the kitchen talking about pooling money for Davey's Dogs, there are three small containers of milk on the table which have tape covering the brand names.

Other mistake: When Roy and Johnny are with the victim who has a projectile in his heart from a lawn mower, Brackett asks for an EKG strip and there's a closeup of the Datascope monitor, even though the Datascope is not actually in the ER's base station room (it's also still in the squad at this point). Also interesting to note, when Brackett walks out of the base station and tells Early about the heart wound, Early turns the medical chart he's holding at an angle, looks down at it and it appears that he's reading his line from the chart. Then when the ambulance arrives at the scene, the stethoscope is around Johnny's neck, then it's back around Roy's neck as Johnny's lifting the patient, then it's gone from Roy's neck, and then Johnny puts it back around Roy's neck.

Continuity mistake: When the patient with the heart wound is brought to the ER, he's wheeled into treatment 2, which has a window on its door, and is on the side of the hallway with the yellow line closest to it. Later we see treatment 1 twice, when Vince brings in the burglary suspect with a bullet in his leg, and also when John and Roy rush the baby to the ER. They used the same room as treatment 1 and treatment 2, but there are just two treatment rooms on that side of the hallway and only one of them has a window on its door - the one that's opposite the nurses' station, treatment 1.

Visible crew/equipment: During Johnny's surgery, the top of the set is visible over the door area when the camera angle tilts up, just before Morton comments that there's blood and wonders where it's coming from.

Visible crew/equipment: After they load Johnny into the ambulance, just as it's about to pull away the reflection of the location's array lighting is visible on the back of the ambulance.

Visible crew/equipment: At the site of the hang gliding accident, when Captain Stanley and Marco are pulling the stokes up the side of the cliff, the outlines of the lav mic transmitter and wire are visible on the back of Cap's shirt.

Visible crew/equipment: After Brackett examines the self-defense instructor, when Brackett speaks to Mrs. Grady in the hallway, the outline of the mic transmitter is visible at the front of Roy's shirt.

Trivia: Throughout the run of the series, when there's a closeup of Squad 51's Motorola two-way radio unit, notice the spelling mistake on the self-adhesive label that reads "Resue Unit No. 51."

Audio problem: When Roy and John are with the unconscious man during the fire at the Atlas Hotel, Chet and Marco bring the stokes and Chet tells them that the fire is getting worse, and when Roy responds, "If we move him now we're gonna lose him," we see that Roy continues talking as Chet listens to him, but we hear nothing.

Visible crew/equipment: While the guys are sleeping at 12:47 am, the camera moves across the dorm, and just as the camera moves past the brick wall divider beside Roy's bed, it casts a moving shadow at the top.

Visible crew/equipment: After Roy and Johnny get back to the station the other guys tell them to check the dog to see if he's sick, and when Roy checks Henry's pupils and Johnny checks his pulse, all the guys are standing behind Johnny and Roy., and we can see the shadows of crew movement in the kitchen at the top right corner of the screen.

Continuity mistake: At the structure fire, before Roy and Johnny go back through the window to look for the missing doctor, Johnny's and Roy's SCBA cylinders have no stickers, but when Johnny's cutting the door, his tank now has two white stickers and Roy's tank has one on the side, then when they're back outside the stickers are gone.

Revealing mistake: At the site of the hang gliding accident, just after the glider falls, in the long shot when Johnny and Chet pull the unconscious victim higher on the cliff, the victim is supposed to be unconscious, yet we see him use his right leg to push himself higher up to help John and Chet pull him up.
