Grey's Anatomy (2005)

(As We Know It) (Part II) - S2-E18

Continuity mistake: The scene where the bomb explodes shows the bomb squad captain walking toward the end of a hallway. Just as the bomb explodes you can see he is about 5-6 feet away from the end of the hallway where another bomb squad tech is waiting. After the explosion the scene cuts to Meredith flying backwards then to a shot of windows being blown out then back to a rear shot of Meredith hitting the floor. Notice here that a second floor is not visible at the end of the hallway. The scene finishes with a final aftermath shot of the hallway as the debris settles. Here we see an entirely different hallway with a visible second floor and the end of the hallway extended by about 10 feet.

Rush Hour (1998)

Continuity mistake: After Carter and Lee enter Azteca Pool Billiards near the beginning, Carter busts the old man for having "cigga-weed". Watch the joint in Carter's hand as he gives the old man a hard time, it changes shape and becomes longer between shots.

Sky High (2005)

Revealing mistake: There are multiple times throughout the movie when the "wind effect", the hair of the characters being blown up by wind off of Speed's super speed, is not in sync and occurs prior to Speed actually zooming past. The 2 most notable occurrences of this are when Speed steals the pudding from Ethan and moves next to Lash and when Speed steals the Pacifier from the secret sanctum.

Crimson Tide (1995)

Deliberate mistake: During the part of the movie where the USS Alabama is sinking deeper into the ocean a shot of the depth gauge was made showing the sub passing 1800 feet at a quick rate. About 20-30 seconds of shots were shown after this to further dramatize the peril of submarine and then the scene cut back to the depth gauge. This time however, the gauge was moving much slower and only shown the sub had sunk 15-20 more feet without any reason for the sudden slowdown in decent.

Crimson Tide (1995)

Plot hole: Throughout the movie an incoming EAM must go through an authentication process prior to it being considered valid. The crew failed to do this on the second EAM of the movie that set DEFCON 3.
