Mr. Monk and His Biggest Fan - S6-E1

Plot hole: It's out of character for Monk, a chronic stickler for details, not to correct an error, especially one relating to his own life. Marcy's diorama for "Mr. Monk & the Three Pies" includes Natalie, who wasn't around yet when "Three Pies" occurred. The doll should be of Sharona, but since that would ruin the ensuing joke, Monk is allowed to overlook it. He's aware of Marcy's unreasoning obsessions, but Monk's OCD would simply not permit him to keep quiet about a mistake that glaring.

A Bullet for El Diablo - S6-E10

Plot hole: During his investigation, Danno learns that the kidnapped Maria never wears make-up. Yet in every single shot we see of her, Maria is far from cosmetics-free. She's wearing very heavy eye make-up. (00:34:40)

The Clock Struck Twelve - S5-E12

Plot hole: As a security test, McGarrett secretly has several dummy bombs planted in the court building, then later shows the governor where they are. Each time he pulls one of the devices from its hiding place, citizens in the courthouse stroll casually by without so much as glancing at the loudly-ticking "bomb." No one knew about the test, so McGarrett's bombs should be attracting some attention, if not causing outright panic. (00:05:00)

The Beacon - S1-E26

Plot hole: Barrows introduces himself as "Dr. Dennis Barrows," and Teddy asks, "What's a doctor?" As the isolated villagers have never heard of doctors, Teddy would have no way to know that "Doctor" was a title/profession and not simply part of Barrows' name. He should have asked, "What kind of a name is Doctor?"

Plot hole: After a chance meeting in Mexico, Tom Matthews invites Kelly and Scott to his home for dinner. The address is unlisted, neither knows where the other lives or is staying, and no address or phone information is exchanged before they part. But Kelly and Scott somehow find the house anyway. Plot hole: No one in Goldman's house finds it at all strange that a mailman arrives at the door with a letter after midnight on Christmas Eve. Nor, a short time later, does the doctor or anyone else passing by at this very odd hour wonder why the same mailman is collecting mail from a street box at dawn on Christmas Day.

Enter Batgirl, Exit Penguin - S3-E1

Plot hole: When he abducts Alfred, believing him to be a minister, Penguin suffers a peculiar memory lapse. He's encountered Alfred twice before in previous episodes, even kidnapped him once before (knowing full well that he was Bruce Wayne's butler). Yet here, he fails to recognize Alfred at all. The Bird's Last Jest (2) - S2-E28

Plot hole: Batman proudly tells Robin that he knows Penguin's men are out of ammo because he's been counting all the bullets. But he'd have no way to know how many rounds they had to begin with, so he couldn't possibly know that they've run out. (00:05:30)

Plot hole: Victoria asks her rescuers to take her to Rancho Velásquez. When they ask where it is, she says, "About 30 miles from here." Though she never indicates a direction, they reply, "Sorry, we're not going that way." (00:23:20)

Quantum Leap (1989)

Another Mother - September 30, 1981 - S2-E13

Plot hole: Sam wallops both kidnappers and rescues the son. They then leave the two child molesters unconscious beside their van. No one calls the police or has them arrested, so they're free to kidnap another kid. Sam doesn't leap until after an epilogue in which the loose ends are supposed to be resolved - but this question is left unanswered. It's not excused by a "just because we didn't see it happen doesn't mean it didn't" defense. The viewers shouldn't be left with a dangling plot hole, and one line could have cured this one.

Plot hole: "Showdown": Darien's lost hat and jacket reappear without explanation after Kane finds him wandering in the desert. Also unexplained is how Kane conveniently happens to be leading an extra, unnecessary horse - unnecessary, at least, until he stumbles across the stranded Darien. (00:15:00)

Plot hole: 3-11 "The Reckoning": Vincent reveals the underground kingdom to Diana for what is meant to be the first time. But she was already there twice in the previous episode, which was definitely not shown out of chronological order. She was taken below by the helpers to meet with Father, and was there again at the end when Vincent christened his son "Jacob." (00:08:10)

Plot hole: "Night of the Savage": Lambert tosses both Selma and his PPT weapon to the floor of the printing warehouse. After Leeds is killed, Lambert retrieves Selma, but leaves without recovering his PPT, a weapon of the future that he'd never leave behind. (00:43:00)

Plot hole: "Cool Hand Darien": In the swamp, Luther overpowers both guards and escapes. As they were both out cold and no one else was around, he could easily have taken the keys from them and unlocked his leg shackles. Instead, assumedly because it's more dramatic, he sneaks back into the prison compound to steal a sledgehammer and break the chain. (00:34:30)

Plot hole: Dr. Linsman puts a heart monitor on Nick for the experiment, but for some bizarre reason, never questions why her "patient" has a resting heart rate of only one beat every ten minutes.

Plot hole: All those years in the spy biz as U.N.C.L.E.'s very famous top agents, and Solo and Illya are still able to successfully go undercover with little or no disguise. Here, all Illya needs is a phony mustache and an even phonier accent. And of course (gotta love it), the Thrush bad guys still don't recognize him. (00:49:00)

Thou Shalt Not Steele - S1-E5

Plot hole: Laura successfully knocks out both bad guys, steals their van and crashes through the warehouse doors. The van doesn't stall, but inexplicably, she stops it, gets out and runs away on foot, even though the baddies are coming to and coming after her. Not the option you'd expect a kidnap victim to choose: most people would floor it and keep on driving. (00:23:40)

Beast Within - S1-E6

Plot hole: The mob hitmen are about to enter the Sweeny family's house to kill them when the police van arrives to move them to a witness protection safe house. In the next scene, the bad guys abduct Catherine and try to force her to reveal where the Sweenies have been taken. Why? They could very easily have waited and simply followed the van. (00:36:30)

Monk (2002)

Mr. Monk Is on the Run (1) - S6-E15

Plot hole: *Spoilers* Monk and Stottlemeyer cook up an ingenious plan to make Monk appear dead, but how the fugitive Monk acquires his regular clothes (in place of Mitch's uniform), the bullet-proof vest and the gun is not addressed. The police are watching Monk's apartment, Disher is watching Natalie and Julie, and Sheriff Rollins is watching Stottlemeyer, leaving us to wonder just how they managed all this right under their fellow cops' noses. Upvote valid corrections to help move entries into the corrections section.

Suggested correction: Stottlemeyer is a police captain. The sheriff can't watch his every move, and there are still plenty of cops who are loyal enough to Stottlemeyer for him to get one of them to deliver them to Monk or leave the items somewhere for him to retrieve.

Captain Defenestrator

Nice theory, but there's absolutely no hint of it in the episode. The lack of any explanation creates a bit of a plot hole because it leaves us wondering.
