The Man Who Lost Himself - S1-E9

Continuity mistake: Blake grabs the money and escapes by crashing through a window. When the bad guys give chase and climb out the same window moments later, the broken glass and framework on the ground is in a completely different pattern than it was when Blake left the scene just a few shots before. Bullseye - S2-E4

Revealing mistake: The secretary turns on the tape recorder to play back her employer's dictation. His voice starts speaking, but the tape reels aren't turning, and the button she pushed hasn't stayed depressed. Apparently, the machine isn't even on. (00:01:20)

Continuity mistake: Seymour watches in horror as the wino is run over by the train. In the long shots, Seymour's hands are waving up and down at his sides. In closer shots, however, he's holding his hands up to his face and screaming.

Plot hole: After stressing (as Waverly did earlier) that only a laser beam can activate the explosive crystals, LeGallows demonstrates the destructive power of one - using only sunlight. (00:37:15)

The Decapod - S2-E3

Revealing mistake: In the midst of Steed's conversation with the wrestling referee, the camera loses focus and suddenly whip-pans away when it shouldn't. The actors continue their lines as the camera comes quickly back and refocuses on them. (00:19:45)

The Atlantis Affair - S1-E9

Continuity mistake: Mark, April and the professor leap from a boat seconds before a bomb goes off aboard. But in the shot of the boat exploding right after they jump, there's no one in the perfectly calm water, and the empty rubber raft is still alongside the burning boat. Somehow, though, the escapees are rowing to safety in that same raft a few shots later. (00:03:20)

Revealing mistake: The motorcycle chase is supposed to take place late at night. But in several shots, a cloud-strewn blue daytime sky can be seen, and long shadows on the streets indicate that the scene was actually shot either in the early morning or the late afternoon. (00:24:45)

The Dog-Gone Affair - S1-E1

Continuity mistake: Though she and Mark have both been calling Putzi the dachshund (who is visibly female) a "she" and a "good girl" throughout the episode, April has an odd memory lapse later and refers to the dog as "he." (00:29:30)

Catspaw - S2-E18

Continuity mistake: Mundy shoots a palace guard with a tranquilizer dart, and the man falls unconscious on the stairs. When his fellow guards find him moments later, the unconscious man has somehow moved off the stairs and is now lying in the corridor. (00:39:15)

The Montori Device Affair - S1-E5

Continuity mistake: The bad-guy bellman knocks Mark out, then zips him face down into a garment bag. He then places the bag, without turning it, onto his luggage cart, and the outline of the body inside (a dummy, judging by how effortlessly he lifts it), is now facing up. (00:22:50)

The Vanishing Lady - S1-E3

Continuity mistake: Blake commandeers the kidnappers' motor home, gets behind the wheel and has to push aside a curtain obscuring the windshield. Then the camera view switches to an exterior shot of the RV driving away - and the curtain, which should still be bunched in the window corner as it was in the interior shot, has vanished altogether. (00:41:15)

The Prisoner of Zalamar Affair - S1-E2

Revealing mistake: Mark finds the bomb planted under his Jeep and puts it under the bad guy's car instead. When it explodes, there's a huge cloud of smoke, but before the shot cuts, you can see that the car is completely undamaged. (00:10:30)

Hawaii Five-O (1968)

... And They Painted Daisies On His Coffin - S1-E7

Continuity mistake: McGarrett strong-arms Tommy-Tommy, the Hawaiian fence, on the beach, and the guy gets sand all over his face during their scuffle. When McGarrett pulls him to his feet, the sand is still there. But in the next shot, all but a small patch near his temple has disappeared. (00:32:40)

The Mother Muffin Affair - S1-E3

Revealing mistake: The climactic chase occurs in an amusement park, supposedly in Brighton, England. But several things give away the real filming location, among them an ad for 15c (not 15p) snow cones, two very American-style phone booths, and best of all, a pole proudly sporting both a U.S. flag and a California state flag, visible in the background behind the phone booths. (00:44:15)
