Hawaii Five-O (1968)

Continuity mistake: Audrey tells Gregory Peck that on the night the allied forces liberated Amsterdam, she watched a movie the troops screened on the street. But in the flashback to that same event, she falls gravely ill with an infection and isn't able to make it to the film. (01:22:40)

Continuity mistake: In the hospital scene, Audrey's mother is sitting down with the bed table to her left. We see her start to rise, but when the shot cuts, she's suddenly standing at the bedside on the other side of the table, with no time transition included for her to get there naturally. (01:57:30)

Revealing mistake: McGarrett paces in his office, and when the shot cuts to a close-up of his feet, the actor's blocking mark is visible, chalked on the carpet.

Here Today, Gone Tonight - S5-E17

Continuity mistake: Danno boards the helicopter without his briefcase, but arrives to conduct the interview carrying the case and tape recorder. He takes the copter back home again with nothing in hand, getting into his car empty handed as well. But he enters McGarrett's office carrying the briefcase again.

Death with Father - S6-E19

Revealing mistake: The teaser contains footage of a stunt-gone-wrong. The getaway truck smashes into a mock-up police cruiser that has been secured with a chain to pull it back out of the shot. The chain breaks, and is visible trailing from the bumper as the fake cruiser starts an unplanned roll down the hill. (It went on to collide with and damage two real HPD patrol cars rented for the production).

It Crawled Out of the Woodwork - S1-E11

Factual error: When the energy monster kills his entire research staff by stopping their hearts, Dr. Block brings them all back to life (with their minds and personalities intact) by installing pacemakers. But when the heart stops functioning, brain death occurs within minutes. For this to work, he'd not only have to operate on several people simultaneously, he'd also have to be the fastest heart surgeon on Earth. (00:40:00)

The Invisible Man (2000)

Beholder - S1-E15

Continuity mistake: The assassin rips out a magazine page with Leila's photo on it and sets it ablaze with a cigarette lighter. The face in the picture burns while he's still holding the page. But in the next shot, after he drops it, the face is still intact and starts to burn all over again. (00:22:30)

The Twilight Zone (1959)

The Fever - S1-E17

Continuity mistake: After Franklin falls through the window at the end, he lands on the pavement below - on top of the broken window frame. Next shot, when the police are examining the body, the window frame has moved out from under Franklin and is lying about a foot away. One shot later, though, it's back in its original position underneath him. (00:20:00)

Continuity mistake: When Joey turns off the bathtub tap, we can see that the water is no longer flowing. But the sound of running water continues. And a few shots later, the water is flowing once more, even though Joey hasn't gone near the tub again. (00:24:30)
