Revealing mistake: Mismatched stock footage puts the Venus-bound Barton in four different spaceships during his journey. We see brief shots of a Vanguard rocket launch, an Atlas missile, a V-2 rocket sequence, and finally, special effects shots of the ship borrowed from the 1950s SF series "Men Into Space." Not one of these vehicles even remotely resembles any of the others. (00:23:15)

Continuity mistake: When Hobbes enters Claire's lab, the wall clock (which we can see is running) reads 9:15. Four minutes of continuous scene later, though, it's still 9:15 - and the clock is still running. (00:10:00)

Continuity mistake: When Dr. Stone's assistant helps him up from the floor, there's a lopsided, circular diagram drawn on the chalkboard behind them. One shot later, the diagram has changed into a perfectly round circle and has moved itself several inches higher on the board. (00:06:10)

Trivia: Rod Serling adapted "The Hitch-Hiker" from a 1941 Mercury Theatre on the Air radio play that had originally starred Orson Welles. Playwright Lucille Fletcher was displeased with the result, primarily because Serling changed the gender of the lead character from male to female, naming her after his daughter Nan.

Continuity mistake: After running through the woods getting soaked by pouring rain, Casha arrives on Kollos' doorstep with her hair neatly coiffed and perfectly dry. In fact, within minutes of coming in out of the downpour, Casha, Leonora and Tone all have completely dry hair and clothes. (00:15:00)

Continuity mistake: As Kono talks to Johnny Kalama in the students' apartment, an orange lava lamp sits on the shelf between them. Each time the camera angle reverses, the wax bubbles in the lamp are very noticeably in different positions and configurations. (00:41:00)

Trivia: This is the only Outer Limits episode that lacks both opening and closing narration. It was absent because "Forms" was the unsold pilot for a series called The Unknown. A second version, stripped of all its supernatural elements (Tone was simply a mad scientist, not a real time traveler), also failed to sell.

Other mistake: Mace's chest camera sends images from impossible angles - including several that include Mace himself. At the end, his superiors watch him from a camera that's obviously mounted in a tree and not on his person. Yet when Mace smashes his chest camera, the transmission goes dead anyway. (00:48:00)

Continuity mistake: Danno, posing as a photographer on the beach, snaps the shutter at a bikini-clad girl and quips, "I only wish I had some film." Seconds later, he takes a picture of the suspects that's then used to identify them. The scene is continuous, and at no time did he ever load film into the empty camera, nor did he have another one. Continuity mistake: When the cleaning woman's body is found, she's lying on the floor face up. A short time later, despite repeated admonitions that nothing should be or has been touched, the corpse has somehow turned itself over and is now lying face down. (00:02:05 - 00:07:45)

Continuity mistake: When he introduces himself to Millicent inside the train station, Paul is wearing a dripping wet raincoat. It stays wet in the close-ups, but in the next several inter-cut two-shots, it's completely dry. (00:11:15)

Factual error: The astronauts land on an asteroid said to orbit a binary star 655 million miles from Earth. This is impossible, as that distance would place them well within our solar system. In fact, they'd be inside the orbit of Saturn, where of course there aren't (nor could there be) any extraneous suns. The nearest star to our system is, in fact, trillions (not millions) of miles away. (00:02:15 - 00:04:00)

Revealing mistake: When Franklin leaves the check cashing window and walks to the right, a man with a pompadour haircut walks off screen left. In the next shot, with no time lapse, Franklin returns to his slot machine, and the same extra passes behind him from the right, once again exiting left. No way he could have moved that fast in the time allotted. (00:13:05)

Trivia: The slot machines in this episode had to be obtained from the LAPD's impound lockers, because slots were illegal in California at the time. Said producer Buck Houghton, "There was a policeman on the set at all times, to make damn sure that somebody didn't take one off and set it up in his uncle's barber shop."
