Factual error: When McCauley and Rennish find the body of a previous moon explorer, they play back a message he left on a portable tape recorder. But they shouldn't be able to hear this: there's no atmosphere on the moon to carry the sound waves. Even if there were, their pressurized, self-contained spacesuits would still prevent them from hearing the tape. (00:19:30)

Other mistake: Just after Cally says that the pilot's death was "a misfortune," she stands and her chair scrapes noisily on the floor. The sound apparently startled Paul Darrow, causing the always-unflappable, nerves-of-steel Avon to flinch: the only time in all four seasons that he's ever seen, albeit briefly, to break character.

A Room with a Rack - S2-E21

Continuity mistake: In the dungeon, Scott holds the conquistador helmet in front of him with both hands. The shot cuts, and it has abruptly shifted to his right hand. In the next shot after that, the helmet is in his left hand. (00:42:00)

Audio problem: When Scott tells the Tokyo police that two kidnappers are locked in the van, we hear his voice, but his lips aren't moving. (00:42:00)

Continuity mistake: Kelly puts Scott's red sweater on a stick dummy to fool the sniper, who promptly shoots a hole through it. But after Scott puts the sweater back on, the bullet hole completely disappears. (00:30:20)

Continuity mistake: In the film's final scenes at the fuel storage facility, full sunlight is streaming through the overhead rafters. Moments later, it's completely dark and the plant's floodlights are all turned on. A few shots further on, the lights are off and it's daylight again.

Factual error: At the drive-in movie theater, Cody orders his wife to turn off the speaker. She twists the knob in the wrong direction (to the right, which would have increased the volume instead), but somehow the sound turns off anyway. (00:29:00)

Continuity mistake: At the end, Filer puts his jacket on, and the left side of his shirt collar is sticking out above the coat. Every time the camera angle changes, though, the shirt collar is neatly tucked in. (00:49:00)

Silver Streak (1976)

Continuity mistake: When Devereaux is shot by the cop in the helicopter, he's alone in the Silver Streak's engine compartment, leaning out the open door. He's then hit by an oncoming engine. In later shots of the runaway train, there's no sign of his body or of any blood, and the door has somehow closed itself. (01:44:00)

Character mistake: Before he scampers inside to spy on the humans, Claudius tells the other animals, "I'm your mouse on the inside!" Maybe he should check a mirror while he's in there. Claudius isn't a mouse - he's a rat.

Continuity mistake: Just before Liz rings the doorbell, the wine stain on the front of Jon's shirt changes shape repeatedly between shots, then vanishes altogether. (00:06:30)

Continuity mistake: When George puts on Grover's purple jacket in the men's room, the collar is turned up in back. In the very next shot, it's rolled neatly down again, even though he hasn't touched it. (01:10:00)

Visible crew/equipment: When Sarah drives away from the clinic in tears after seeing Johnny, the side of her car shows us two banks of studio lights, a reflector panel and part of the camera crew. (00:24:00)

Revealing mistake: For most of the opera house stage presentation, the monster is a full head taller than Friedrich. During the tap dance, however, they're the same height. Although the camera angle/depth of field used tries to disguise this by keeping Gene Wilder in the foreground for most of the shots, it's still apparent that Peter Boyle isn't wearing his probably-impossible-to-dance-in platform shoes during the sequence. Continuity mistake: As he's about to depart at the train station, Friedrich leans in to try and kiss Elizabeth good-bye. He's mere inches from her nose when the shot cuts, at which point he's suddenly standing more than a foot away from her. (00:16:00)

Factual error: The huge piece of retrieved space debris breaks loose in the hold and the astronauts struggle to lift and re-secure it. Only they shouldn't have had to. They're in space with no gravity generator, wearing magnetic boots to hold themselves to the deck. The debris would be weightless, too. (00:20:00)

Plot hole: No one in Goldman's house finds it at all strange that a mailman arrives at the door with a letter after midnight on Christmas Eve. Nor, a short time later, does the doctor or anyone else passing by at this very odd hour wonder why the same mailman is collecting mail from a street box at dawn on Christmas Day.

Continuity mistake: In the dining room, Mary places the telephone on the kitchen counter by reaching through the double doors over the bar. The doors, which she pulls shut, bounce back open slightly on their own. Mary then commits suicide. No one else has been there, but when Agatha returns to discover the body, the bar doors are tightly shut again. (00:35:10)
