Factual error: The woman who accuses Monk of stealing her necklace shouldn't have the necklace inside the asylum in the first place. For both safety and insurance reasons, patients in mental facilities aren't permitted to keep any potentially harmful jewelry, including pins, broaches, earrings, belts, beads and necklaces.

Other mistake: 1968: When David and Amy discover Quentin's sealed room, the youthful performers forget to stop on cue and skip to lines from their next scene. The view cuts to Liz and Roger in the drawing room, but the voices of David and Amy excitedly continue from the set next door.

Continuity mistake: It's spring in San Francisco in this episode: warm with light rain and people outside in shirtsleeves. That is, until Monk goes to visit Kelly, when it suddenly becomes winter. Everyone's bundled in heavy coats and it's sleeting. After Monk leaves Kelly's house, it becomes spring again. [The Kelly Street sequence was originally meant to be used in "Mr. Monk and the Candidate, " where everyone is dressed appropriately for cold weather. For whatever reason, it instead got used in this episode.]

Other mistake: 1970: Barnabas fends off a werewolf by striking at it with his silver-headed cane. Unfortunately, the cane bounces off the resilient werewolf and hits Barnabas in the head. (Jonathan Frid reportedly had to have a few stitches afterwards.)

Continuity mistake: After Edith stumbles and nearly falls down the stairs, she still has both her shoes on. But after the kiss, she proceeds up the stairs and is now carrying one shoe. She had no time to take it off, as she was, er, busy with Kirk the whole time. (00:43:55)

Continuity mistake: The blood stains on Nick's white turtleneck are lighter, then darker, then lighter again, and migrate to different positions, both during his fight with LaCroix in the abattoir, and throughout the hospital scenes afterward. (00:10:15 - 00:19:30)

Factual error: 1968: The basement's brick wall crumbles to reveal Trask's skeleton hanging in the alcove. Only it shouldn't be. Bodies do not remain assembled once the flesh and sinew are gone. Trask's remains ought to be a large pile of bones on the floor.

Factual error: The murderer works for a blood bank and drives a bloodmobile, a plot holdover from the first pilot ("Nick Knight"), which was set in Los Angeles. This second pilot, however, is set in Toronto - and Canada doesn't have mobile blood units. (00:31:35)

Suggested correction: I listened to this carefully and it is incorrect. She does, in fact, say Collinwood. The enunciation is just a bit muddled.

Still sounds like "Hollywood" to me. But the correct name, in any case, is "Collinwood," not "Collonwood."

Other mistake: 1968: The now-vampire Angelique kneels beside Barnabas' chair to tell him he belongs to her now. When she tries to get up, her long white gown catches on something, causing her to muff her next line a bit as she gives the fabric an annoyed tug to free it.

Revealing mistake: 1968: When the psychic enters Quentin's haunted room, she stumbles over the wire used to "ghostly" open the door, and has to grab onto the gramophone table to keep from falling. The table almost falls over with her, but she manages to stay upright, barely.

Other mistake: 1795: A black-and-white cat, supposedly the transformed Joshua Collins, was completely uncooperative in every scene he was in. He always started out on a bed or a chair, but got up and left every time, usually forcing the camera to pull in so that the actors could pretend the cat was still there.
