Blake's 7 (1978)

Terminal - S3-E13

Revealing mistake: To conceal their unauthorized entry into the secret lab, Cally very carefully closes and locks the sliding door. But when she turns back to speak with Tarrant, the "secured" door rolls halfway back open by itself. No, it's not Servalan's troops sneaking in to catch them: there's no sign of anyone there. Besides, anyone unlocking the supposedly-locked door would have alerted them. A case of malfunctioning BBC scenery, plain and simple. (00:43:45)

UFO (1970)

Exposed - S1-E2

Revealing mistake: Due to re-used special effects footage, the submersible Sky One fighter, whenever it launched from the ocean at night, always took off under a full moon. (00:07:50)

The Bridge of Lions Affair (2) - S2-E21

Factual error: While Solo prepares the explosive, Waverly's line to Joanna is, "If you and I are wise like Lot's wife, we'd better not look." The scriptwriter got this backwards. Lot's wife was unwise and did look, and turned into a pillar of salt. (00:41:00)

Blake's 7 (1978)

Rescue - S4-E1

Continuity mistake: Tarrant's hair seems to have grown at least an inch overnight. It's longer and bushier than it was in "Terminal," which supposedly took place the day before. (00:09:45)

The Bridge of Lions Affair (2) - S2-E21

Continuity mistake: At the end of Part 1, Sir Norman enters the rejuvenation device and immediately gets younger. As Part 2 begins, however, he emerges from the machine looking the same, and Dr. Gritsky says that his rejuvenation will occur gradually over the next few days. (00:03:30)

Blake's 7 (1978)

Rescue - S4-E1

Continuity mistake: The damaged Orac has long loose wires hanging out when Dorian carries it off the ship, but they disappear in the next shot, when he enters the base. (00:24:40)

Blake's 7 (1978)

Rescue - S4-E1

Continuity mistake: The weather on Terminal has changed from Summer to Winter overnight. When Avon first beams down, it's obviously warm and all the trees have leaves. The very next day, all the trees are bare and there's snow all over the ground. (00:01:30)

Blake's 7 (1978)

Terminal - S3-E13

Visible crew/equipment: As Servalan begins explaining her nefarious plan to the captured Avon, an equipment shadow slides across the blue diamond-shaped pane just behind her. (00:40:30)

The Moonglow Affair - S2-E23

Revealing mistake: Miss Moonglow's lipstick supposedly glows in the dark. But when the lights go out, a badly superimposed special effect causes the glowing lips to remain completely stationary, even when she doesn't. Her head moves, but weirdly, the lips don't. (00:36:20)

The King of Diamonds Affair - S2-E25

Continuity mistake: Delgado's plane makes a bumpy landing (without a runway) and skids to a stop in the Brazilian jungle, surrounded by palm trees. But when everyone gets off, the plane is suddenly parked in the midst of an industrial compound with buildings all around it, and no palm trees in sight. (00:37:45)

The King of Diamonds Affair - S2-E25

Revealing mistake: When Solo and Illya, posing as maintenance workers on the London street, are opening the manhole to go underground, two cars - a blue VW hatchback and a white roadster - pass them twice going in opposite directions each time. And while most of the cars are driving appropriately on the left, one pulls up behind them driving on the right (which is to say, wrong) side of the road. (00:13:00)

The Living Legend (2) - S1-E13

Continuity mistake: After the Colonial assault force parachutes into the Cylon city and Boomer says, "Shouldn't take too much to blow 'em sky high," they race off to the left - and Apollo's sky diving helmet disappears from one shot to the next as they run.

Monk (2002)

Mr. Monk and the Astronaut - S4-E14

Plot hole: The murderer mails a garage door opener with the button taped down to his victim and kills her by remote. But A) With the switch continuously in the "on" position, the battery would die long before the package arrived, and B) Door openers use special frequency tones that are set much like a safe combination. The likelihood of one control opening 3 different doors in the same area, as it does here, is virtually nil. (00:33:30)
