Other mistake: 1967: The death of Sam Evans didn't play out according to the script. That's because the cameraman accidentally knocked the teleprompter off (with a loud crash), causing the dying "Sam" to sit up and yell "Where is it?!" "Maggie" ad libbed, "Never mind, Pop. It's going to be OK." Poor Sam then had to expire without delivering any poignant last words to his grieving daughter.

Other mistake: "Night of the Bubbling Death": When West lifts the unconscious bad guy by the shoulders and prepares to drag him out of sight, you can see drag marks already in the dirt from previous takes. (00:21:10)

Mission to Destiny - S1-E7

Other mistake: Just after Cally says that the pilot's death was "a misfortune," she stands and her chair scrapes noisily on the floor. The sound apparently startled Paul Darrow, causing the always-unflappable, nerves-of-steel Avon to flinch: the only time in all four seasons that he's ever seen, albeit briefly, to break character.

Suggested correction: Dick isn't amazingly precognizant. He's wearing swim trunks while using the rag to prep his own surfboard. Barbara Gordon was already at the beach with Skip, and Dick is heading to the beach with his surfboard as well (00:04:34).

The Ring of Wax (1) - S1-E23

Other mistake: Riddler says that his "universal wax solvent" can eat through anything. Which begs the question, just what is that vat he cooks the stuff up in made of? We see the wax burn through solid steel vault doors. So it also ought to be eating through the vat, the floor and the bedrock, and sizzling its way down to the Earth's core. (00:07:00)

Other mistake: Chambers asks what time it is on Earth, a strangely vague question to which the Kanamits grant an equally odd answer: on Earth, it is noon. It can't be noon everywhere on Earth at once. While it can be fancifully explained away ("They somehow knew to answer for the time zone he was taken from"), it still jars the viewer, because neither Chambers nor the aliens, both highly intelligent, should make this sort of imprecise mistake. Both director Richard Bare and Damon Knight, author of the original short story, confirmed this as a scriptwriting error that should have been caught before shooting began. (00:02:15)

In the Shadow of Z'ha'dum - S2-E16

Other mistake: In this episode, a simple light frequency modulation enables Sheridan to briefly see the Shadow creatures. But because J. Michael Straczynski decided that this was just "too easy," the technique was "accidentally on purpose" forgotten about for subsequent episodes. If the Shadows had somehow learned to adapt and remain invisible, lines to this effect would have been written in. JMS deliberately chose to "conveniently forget" about it instead. (00:40:00)

Other mistake: Mace's chest camera sends images from impossible angles - including several that include Mace himself. At the end, his superiors watch him from a camera that's obviously mounted in a tree and not on his person. Yet when Mace smashes his chest camera, the transmission goes dead anyway. (00:48:00)

Other mistake: Balya, the Turkish representative, is killed by a knife protruding through the back of his chair. But when he falls to the floor, leaving the knife behind, there's no blood on the blade. (00:42:00)

Other mistake: Illya orders an espresso from Fred, the coffee house proprietor, but never pays for it. And Fred doesn't take a bill to his table or speak up when he leaves without paying. (00:10:00)

Whom Gods Destroy - S3-E14

Other mistake: At the end, Kirk teases Spock twice specifically about letting himself be hit on the head in order to determine which Kirk was genuine. But the phony Kirk never hit Spock on the head. He merely pushed Spock over and attacked the real Kirk. Clearly, the action sequence wound up differing from the scripted dialogue, and no one noticed. (Also clearly, Kirk is not speaking metaphorically here.) (00:45:30 - 00:49:20)

Other mistake: Nick pronounces the plastic surgeon's name as "Yergen" in one scene and as "Jergen" in the next. At the end, she becomes Dr. "Yergen" again. (00:29:40 - 00:31:10)

The Foreign Legion Affair - S2-E22

Other mistake: Aboard the plane in the beginning, Illya calls to report in, and forgets his own secret agent number. He tells the communicator, "This is Number 2 Section 1." This is a little like James Bond suddenly calling himself Double-O-Six. Illya worked for Section 2, and was always Number 2 Section 2. Maybe that's just what happens when you're a TV spy and get hit over the head too many times. (00:03:15)
