Continuity mistake: After speaking to Bill Graham in the tent Captain Willard comes back and tells the guys, "I negotiated two barrels of fuel for a couple of hours with the bunnies". As he walks back to the boat the hills in the distance can't be seen because of the rain but in the next shot when Chief Phillips comes over to talk to him the hills can be seen. (01:26:25)

Continuity mistake: There's a shot of two injured soldiers in wheelchairs sitting front row dead center as playmate of the year Carrie Foster is brought out to the audience. There's another soldier crouching between them who disappear between shots. (01:07:50)

Continuity mistake: After he decides to decline the offer to spend some time with the bunnies Chief Phillips picks up some tools off the roof of the boat. In the next shot he's already on the floor of the boat turned around and ready to go down the hatch to work on the engine. (01:27:25)

Continuity mistake: Captain Willard says to Chief Phillips, "how long has that kid been on his boat?" and a long green piece of fabric is dangling off the roof of the boat behind the Chief. In the next shot of the captain, it's gone. (01:19:20)

Continuity mistake: On the boat while Captain Willard reads Kurtz' file Clean starts making noise by tapping his drumsticks. This annoys the captain and as he gets up to confront him we see, at the right of the screen, the helmet that Clean is drumming on. In the next shot the helmet and its strap have been moved around the metal pole that holds up the roof. (01:19:05)

Audio problem: On the boat while reading Kurtz' file Captain Willard narrates to himself, "dear son, I am afraid that both you and your mother well have been worried at not hearing from me during the past weeks, but my situation here has become a difficult one". At this point they show the actual letter that he's reading from and the text of the letter is not the same as what we are hearing. (01:20:40)

Audio problem: Clean listens to his transistor radio as the DJ says, "…and we have an important message for GI's who are living off base from the Mayor of Saigon". The volume that we hear stays the same but watch his thumb on the left of the radio as he turns the volume knob up. (01:14:05)

Continuity mistake: Just before they leave the USO show the helicopter pilot on the left goes from looking at the controls to looking at the camera between shots. (01:10:10)

Continuity mistake: On the beach at Vin Din Dop Captain Willard steals the surfboard from Lt. Col. Kilgore's men and the tail rotor on the lieutenant's helicopter that just flew in has been removed. (00:51:25)

Visible crew/equipment: On the boat while reading Kurtz' file Captain Willard recalls Harrison Ford saying, "and follow every order, however ridiculous" while having lunch with the general earlier in the movie. If you look at the close-up of his eye you can see a cameraman and another person's reflection as they stand in front of him. (01:18:35)

Continuity mistake: A guy from Lazaro's boat throws a smoke canister that catches fire on the roof and Chef tries to put it out with a fire extinguisher. When we first see him trying to put the fire out his shirt is dry then the next time we see him his shirt is full of sweat. (01:16:40)

Continuity mistake: Two other patrol boats pass them on the river going the other way and Chief Phillips yells over the megaphone "is that you Lazaro?". The guy whose pants fall down has a cigarette in his mouth, and his pants are down further in the front as the boats approach. In the next shot after they pass by, he has no cigarette in his mouth. Continuity mistake: In a close-up Chief Phillips he says, "sampan off the port bow" and we see a kaki green T-shirt hanging next to him at the right of the screen. A couple of shots later Captain Willard says, "what up chief" and Phillips takes the T-shirt down. In the next wide shot of the boat the T-shirt is back. (01:34:15)

Continuity mistake: As Chef tries to put the fire out with the fire extinguisher Captain Willard is hanging over the side of the boat with Clean as they try to escape the flames. In the next shot and as chief Phillips walks around him he's behind the helm of the boat near the Playboy picture that Chef taped up. (01:16:50)

Continuity mistake: Captain Willard sits in the sunshine near the back of the boat reading his file on Colonel Kurtz but in the wide shots of the boat as Lance is water skiing he's not there. (01:15:20)

Continuity mistake: Captain Willard says, "the army tried one last time to bring him back into the fold" and we see a close-up of General Corman again sitting at lunch with the captain from earlier in the movie. This was not shot at the same time because the general's shirt is different. Look closely at his name, the collar covers everything except the first two letters whereas earlier in the movie, for the entire scene his name was showing and could be read. (01:18:10)

Continuity mistake: On the boat Chief Phillips says to Captain Willard, "my orders say I'm not supposed to know where I'm taking this boat, so I don't". If you look at the binoculars next to Chief Phillips on the right of the screen they rotate 90 degrees between shots. (01:19:40)

Continuity mistake: As they leave the MedEvac we see an overhead shot of the boat and the flag has changed. The back of the flag has been cut up considerably since when they arrived at the MedEvac even though they haven't been it any battles. (01:32:55)

Continuity mistake: As they approach the Do Lung Bridge Chef talks to Lance with his gun muzzle pointing out in front of him. The muzzle isn't near anything but in the next shot it's resting on the flat piece of metal on top of the machine gun. (01:40:35)

Continuity mistake: Chef asks his playmate, Miss May, to put on a black wig as they sit in the helicopter. When he first reaches for the wig it's on top of a Styrofoam head but in the next shot he reaches for it again and it's on a small table to the right of the Styrofoam head. (01:28:50)
