Six Feet Under (2001)

Pilot - S1-E1

Continuity mistake: Right at the end when Nate is looking at people passing by on the street, his facial hair is thicker in some shots than in others. This is because they had some reshoots, as explained by Alan Ball in the commentary.

Revealing mistake: Near the end, there's a slow motion shot of Sam and Mikaela running with explosions behind them. If you look at Mikaela closely, some fire goes right through her (this was added in post-production and they didn't rotoscope her properly).

Entourage (2004)

The WeHo Ho - S4-E6

Visible crew/equipment: When Turtle is in the kitchen talking to the guys about his cousin, we have a tracking shot of him going to the table and then returning to the bar. If you look at Vincent's face during this shot, you can see the cameraman's shadow as he passes him.

Entourage (2004)

The Return of the King - S3-E17

Continuity mistake: When the boys come back from the track it is implied they bought a horse and attached a horse carrier to their car for it. You see it in an outside shot. But when they're filmed inside talking, you can see a lot of the rear window of the car and no horse carrier is seen.

The Departed (2006)

Visible crew/equipment: When the song starts, right before the main title, we have a dolly shot of Billy in jail. It starts from a red wall and moves towards him. If you look closely at the wall, you can see the subtle shadow of the camera and the rest of the equipment as it moves.

Go (1999)

Visible crew/equipment: When Adam says he has orange juice in the car, the camera does a fast pan towards Ronna. If you look on the upper left corner, you can see the shadow of the boom for a second or two.

Happy Gilmore (1996)

Continuity mistake: When Happy hits the ball during the bet with the two movers, we see a point of view shot of the ball traveling towards a house. When we get a shot of the owner of that house being hit, it's a different house.
