Continuity mistake: When Barr is being interviewed in the hospital, at the end of the film, the bloodiness of his facial injuries lessens as the scene goes. Also, it seems like the prosthetic scab on his top lip is coming loose. (02:02:00)

Factual error: When Reacher sees Sandy's body being loaded into the coroner's van, he's able to see her face. In reality, a dead body is zipped into a leak proof body bag for transport. And that body is put in the bag and zipped up WHERE THEY DIED, not in the van or somewhere else. Specifically, this was a murder scene and any evidence and DNA would have been secured on the body by the body bag.
Factual error: At the shooting range when Reacher hangs his target, you can see it's either AS500 or something softer because of the damage. When Reacher fires his shots go clean through the target without any spalling damage done to the paper.
Revealing mistake: In the car chase, the stunt ramps on the ground so that both cars can easily race over the curbs instead of destroying their wheels. (01:20:20)
Continuity mistake: In the car chase scene in Jeb's car after Sandy is killed, Jeb's left rear tail light and front right fog light go from working to not working to working again over and over.
Continuity mistake: In the pursuit scene between Jack in the red Chevy and the two bad guys in the silver Audi, the Chevy right front fender is completely wrinkled after a side to side crash. When the car stops after exiting the tunnel, the fender is still wrinkled, but when Jack momentarily loses control and the car skids, the fender is unwrinkled and stays that way.
