Correction: There are barely any shots with the camera in someone's glasses. If you want to submit a visible equipment mistake, be more specific.

Corrected entry: In the middle of the film when we see Cypher talking to Trinity on the phone, at the end of their conversation we see Tank with the big electric gun, when he fires it, pause and use frame by frame or slow motion and you can see its not Cypher (Joe Pantoliano) but his stunt double, this can be seen because the brightness of the gun causes his face to be bright so you can see the stunt double. Corrected entry: Keanu Reeves wanted to do his own stunts, but was disappointed when he learned that his neck injury would limit his ability to do the shots. So they only managed to get about 3 takes with Reeves actually in them, and then used a body double and CGI.

Correction: Please cite a source for this, as in all interviews and material the cast (including Reeves) is known to have done the majority of their fighting and stunt work. Reeves' injury did delay his martial arts training, but it did not prevent him from performing.

Corrected entry: While explaining the Construct, Morpheus tells Neo, "Your appearance now is what we call residual self image. It is the mental projection of your digital self." What he really meant is, "It is the digital projection of your mental self." (00:40:00)

Correction: The concept of a mental projection of one's digital self is rooted in the idea that the person's memory of how they looked (in this case, curated through years of living in an illusory digital reality) is going to manifest itself within any simulation that allows it. Because Neo lived in the matrix, he has a mental image of what he should look like (his digital self). So the line in the movie is correct.

No, it is not correct. As explained by others above (and overwhelmingly upvoted by other site users), it would still be a digital projection of your mental self. There actually is no digital self. It's just a digital projection. It is what is in the mind that is the self and what the film values as the core of an individual. "The body cannot live without the mind," as Morpheus says. Also, it is in the mind where we believe. Throughout the film, the mind is central. Thus, the correction stands.

Those living in the Matrix are digital representation of themselves. Which is why Neo has hair in the Matrix but is bald in the real world. When in the construct, Neo has hair again because that's how he remembers himself, which was his digital self, not his real self.

Correction: That is not a mistake, Morpheus spoke correctly. They are in the construct, therefore it is Neo's digital self. His clothes and hair are his mental projection. Just as if Neo were dreaming it would be: "It is the mental projection of your dreaming self."

No, they are in the construct, a digital environment. So the projection is digital (being that it exists in a digital environment) and the self is mental (being that it is what he thinks it should be). The correction of the movie quote is correct.

When in the Construct, it's like being connected to the Matrix. The people in the Construct are Avatars of themselves, which means they are their digital selves. The projection of themselves is based on what their Avatars are thinking.

That is pure nonsense. Avatars don't think. Your comment makes no sense. As explained elsewhere, it should be "the digital projection of your mental self."

I probably oversimplified what I was trying to say. But I can understand your confusion. The digital self is what a person looks like in the Matrix, which is different from the real world. The mental projection (in the construct or as an avatar) resembles their digital self, not their real self. Which is why Neo in the Matrix has hair, real life Neo is bald, but his mental projection has hair.

Other mistake: They are presumably in the US - trying to make it look that way, anyway. Cars driving on the right, speaking English etc. However the filming location was overseas, revealed by the elevator in the lobby scene. When they hit the button, the plate refers to the elevator as a lift.

Suggested correction: As pointed out in several other mistakes, The Matrix is a computer simulation and is not meant to be a perfect model of the world. Small things like this are possible.

Corrected entry: Cypher alerts the agents to the location of Morpheus' base of operations via cell phone tracing. But since the agents can transport to any body in the Matrix that hasn't been unhooked, and they can trace calls in about 30 seconds as we saw at the start, it would have been easier if they had transported to the nearest bodies, got into the building, killed Mouse, destroyed their equipment and then summon the police. All this they could have done in 5 minutes while everyone was at the Oracle, but instead we get a very long, action packed confrontation.

Correction: The agents made a different call, they wanted to trap the entire party but their primary target was Morpheus and they had to make sure he wouldn't get away. Anything else they tried could cause Morpheus to escape.

She turns slowly in the air, eventually facing upwards when she crashes through the window, no longer turning. Suggested correction: She isn't facing up, if you look the camera does a 180° around Trinity, and also if she appears to be facing the top of the screen, she's actually still looking at the street which in this moment is... well, above the screen.

Suggested correction: Actually, I think that's correct and reflects how the agent 'took control' of his body. When he took on the pilot's form the pilot turns to the side in agony but the agents face actually appears from the side of his skull. So being shot in the temple then, would be consistent with a shot either to the front or the back of the head. In considering that the victim turn his head to the right when he was 'transformed' and given that agent was shot in the left temple, it would imply that the victim was actually shot in the back of the head? The would we saw should've been an exit wound, but it appeared to be an entry wound.

Visible crew/equipment: When Neo is in the car and asking Trinity if she's seen the Oracle, the white reflective screen is visible in her glasses where the windshield should be.

He is called Thomas at the beginning of the movie, when he doubts the truth -- that the world as he knows it is not real. Thomas is the "doubting" disciple in the Bible.
