Mistake Other: In the scene where the mirror cuts his hand, Ben touches the mirror with his left hand. In the very next shot he is holding the flashlight with his left, pointing on his wounded right hand. Right after that he switches hands again.

Mistake Factual error: In the scene after the mirror-person rips the sister's jaw apart in the bathtub, the detective in the following scene drains the water from the tub by removing the drain plug. No detective would have done this, as there could be evidence of the cause of death and the murderer (if any), and they would drain the tub with buckets and sift through them to preserve anything they find.

Mistake Continuity: When Amy Smart gets into the bathtub after looking at herself in the mirror, there's very little foam on the water. Then there's a cut back to the mirror and in the following shot there's suddenly a lot more foam.

Updated today Plot hole: Water counted as something that would give a reflection good enough to let the person in the mirrors come, but the person could not use other things, for example people's eyes or clean knives, when they gave better reflections than water or very close to it. (This is a plot hole because it is never explained.)

Mistake Other: At the end of the film, when Ben is in the mirror world, all the writing on the police cars is backwards. However the word "Police" on the car's bonnet is also backwards. Since in the regular world this is already written backwards to be read from a car's rear view mirror, this word should appear written normally.

Mistake Revealing: At the beginning of Angela's bathroom death scene she is looking at herself in the mirror. You not only see her reflection in the mirror, but also in the shiny metal strip holding the mirror at the bottom which is to be expected. When she turns away and her 'demon' reflection is watching her, you still see the image in the metal strip. As the 'demon' is in the mirror you should no longer see anything reflected in the metal. This does seem to show how these moments were filmed, with the real actors reflections being recorded.

Mistake Continuity: In the scene where Ben is taking the pills and has the conversation with his sister, she leaves and he takes a drink. When he raises his head and sees his distorted face and screams, his sister rushes in and when she leaves after getting screamed at, she begins to walk out and never touches the door. In the following shot of Ben getting up, the door is closed, with his sister never having touched it. Nor is there the sound of it closing.

Mistake Other: In the scene where the son is woken up by the "screaming woman", when the mother rushes out of her room you can see the sun in the window behind her, despite the the fact the scene takes place at night.

Mistake Other: After imagining himself on fire, Ben rubs his hands all over his white shirt. Despite the fact that his hand was recently cut, there is no blood transferred to his shirt.
