Continuity mistake: Watch the light when Matthews is breaking the bone so that he can slide his foot out of the lock (right after he's done smashing it with the lid). The flashlight is suppose to be on the floor next to the foot (it was in the previous shot) but the light source is moving all around as he places his hands on his foot and then breaks it. It's very noticeable as soon as the break happens, the light source moves down and to the left of the screen. The flashlight is back on the ground, next to the foot, in the very next shot. Continuity mistake: When Eric slams Amanda to the wall she eventually starts bleeding profusely. But you can see that there are no blood spatters on the wall even when he continually tries to do this a few more times after that.

Continuity mistake: When Amanda is dying in the end, the amount of blood on her mouth and coming out of her mouth changes between shots. More than once the trickling blood disappears then comes out again.

Continuity mistake: As Jigsaw is dying his bed is mostly reclined and his head is lying flat, but when you see the last shot of him you can see that the head of his bed is more elevated and he's slumped.

Continuity mistake: When Amanda gets shot and falls next to the cart you can see that there is no blood on the cart. The next time you see her there are blood splatters all over it. Every time you see the cart there is a different design of blood on it, and at least one time there is no blood to be seen.

Continuity mistake: In "The Rack" room you get a shot of the box holding the shotgun and the key. When first viewed from the back the keyfob and the key are directly parallel to the front and back of the box. The scene cuts back to the guy in the rack, then back to the box. Now the key and keyfob have moved and are almost vertical to the sides (the judge hadn't started to move it yet, there's nothing to explain how or why it would have moved).

Continuity mistake: When Amanda goes to take off Adam's boots (in the flashback) his feet are spread a good 2, 2 and a half feet apart, yet when she goes for his right boot his feet are all of a sudden less than a foot apart. (01:11:30)

Continuity mistake: The red "blood" marks on Jigsaw's left hand change several times (smaller, larger, smaller, larger again) during the bathroom flashback. (01:11:10)

Continuity mistake: Jeff's body position moves when he's looking down the hall towards the wrecked trike. When the shot is in front of him his right leg is farther forward than his left and he's leaning forwards peering into the darkness, when the shot is behind him he is standing up straight and his feet are parallel.

Continuity mistake: During the scene when Lynn is telling Amanda that Jigsaw needs to have an operation Lynn is talking to Amanda with her body slanted forward and her head and chin up (in a "you're stupid, this is what we need" type of pose), but in all of the over-Lynn's-shoulder shots her head is tilted down to look at Amanda.

Continuity mistake: There is a shot when Tony (chained man) is listening to Jigsaw's message where blood drops out of his mouth and runs down his chin, but in the next close-up of Tony you can see that most of the blood has mysteriously disappeared (he still has blood on his chin, but it looks the same way that it did BEFORE he drooled blood all over the place). Continuity mistake: When Kerry reaches up the second time to try and get the key you can see the acid burns (a few, but not a whole lot of blood) on her hand. There is a quick cut where her hand is poised over the jar of acid and her hand has suddenly become almost entirely red and bloody. (00:06:10)

Continuity mistake: When Amanda dies, her left arm is outstretched and her hand lying vertically on it's left side. There is a quick cut to her body during Jigsaw's final message and you can see that her hand is lying flat on the floor, then it cuts to a close-up of her hand and it's back on its side again. (look at her watch for a reference point to her hand position).

Continuity mistake: When Amanda tells Lynn "Let me give you the simple version" there are several hairs that are across Lynn's face, the next shot of Lynn has her hair back nice and neat. (00:27:35)

Continuity mistake: When Kerry is talking to Rigg and the detective she is almost directly in front of the door. When the other girl says "You won't believe this, we got the tape out clean" the camera pans around and you can see that Kerry is now in front of the chalk boards. (00:11:05)

Continuity mistake: When Kerry is sneaking up to the door, you can see on the TV that she raises her gun up to her chest, but then the shot cuts away (instant cut) and she's still aiming the gun at the closet. (00:13:00)

Continuity mistake: A little bit after you first see Jeff there is a shot of Amanda looking into Jigsaw's room. Lynn is telling Jigsaw to look at her. The pillow is about three-quarters of the way down Jigsaw's head, almost down to his neck. The shot cuts to a closer view of him and now the pillow in a good 8-10 inches higher on his head, up past his ears. (00:36:20)

Continuity mistake: After examining Jigsaw, Lynn tells Amanda that he needs to go to a hospital. Amanda grabs Lynn's hair and pulls Lynn towards her to yell at her. In the shots of Amanda's face you can see that their foreheads are touching, but in the shots of Lynn's face their temples are the only thing that is touching. (00:37:25)

Continuity mistake: When Lynn first gets rolled into Jigsaw's room she has a few strands of her hair in the corner of her mouth. The strands appear and disappear during the next few shots.

Trivia: The camera moves from Troy's crime scene to Kerry in the bathtub in one shot - Dina Meyer had to run around the set, undress, and jump into the tub. If you look closely, you can still see the water moving from when she jumped in.
