Continuity: When Lynn is tied to the chair her hair moves throughout the whole scene.
Deliberate "mistake": In Saw III we learn that Amanda was the person who abducted Adam from his apartment. However, if you watch his abduction in the first Saw film, you'll see that the person attacking him has the hands of a middle aged man.
Continuity: When Jeff sees the mini Jigsaw puppet on the tricycle, the handlebars are just red in one shot, but then they turn grey.
Visible crew/equipment: When Jeff is stuck to the fridge, you can see a cameraman's shadow on his shirt.
Visible crew/equipment: When the figure grabs Kerry, the buttons on the TV reflect a crew member and set light.
Continuity: The tear on Jeff's cheek briefly switches sides when he is holding the third card.
Revealing: When Jigsaw is coughing up blood, the tube pumping the blood is visible. Look near his thigh. You can even see some red liquid oozing up. (Only visible in widescreen.)
Revealing: After John shoots Amanda she falls to the ground. You can tell it is a dummy, just watch her arms and head.
Visible crew/equipment: Just before Amanda shoots Lynn, a mini microphone can be seen on Amanda's neck. It is only visible for a few seconds then vanishes, so look quickly.
Other: Near the end of the film when Jigsaw has the tape player, the 'play' button is near the left side. But Jigsaw's finger is on the right side, not even close to the play button. Jigsaw presses on the right side where there isn't even a button, but the tape begins to play anyway although nobody pressed the actual play button.
Continuity: The model of gun Amanda has when she shoots Lynn changes between shots.
Continuity: Jeff's saw that he used to slit Jigsaw's throat is drenched in blood. Throughout the scene the blood stain gets shorter and longer.
Revealing: When Eric Matthews is smashing his foot, he hits it the first time and it shows a close up. If you watch the small amount of blood when he hits it again they have used the same shot.
Continuity: Just before Amanda shoots Lynn her gun switches from her right hand to her left.
Continuity: The drill that Lynn uses on Jigsaw's skull has a white piece of tape on the end of the drill bit a short distance from the end, presumably to mark the thickness of his skull so she knows where to stop. In one shot (during the drilling scene) the piece of tape moves a good two inches farther up on the drill bit, but it's back down in the very next shot.
Continuity: Right before Lynn says "I don't know what you know, but my marriage has survived more suffering that you could possibly imagine" her hair is all pulled back behind her ear. As she's saying that line you can now see that a noticeable amount is hanging on the side of her face and across her ear.
Continuity: Jeff sees the 3rd tape player (in the room where he finds the judge) and there's a shot of the player being held up by string. The player is hanging very level and the string is perfectly tied around the middle of it. Jeff climbs the stairs to get the player and now you can see that the player is hanging crooked and the string (where it's tied) is slanted towards the top of the player. (He didn't shake it lose or anything when he climbed the steps, the player is hanging from the ceiling).
Continuity: The tear in Jeff's face (after he gets stuck to the refrigeration machine) continually changes throughout the movie. It goes from oozing and bloody, to clean and almost normal looking several different times. There's no blood on his bathrobe, so he didn't wipe it off that way. That's a big enough tear to bleed a decent amount, so we'd see the blood if he wiped it off.
Continuity: When Lynn says "Please, please let me go" there are tears in both eyes and tear marks on her cheeks. 2 shots later (maybe 7 seconds) you see a close-up of her face and there are no tears or tear marks. (Jigsaw was holding her hand, so she couldn't have wiped them away, plus you can see Lynn the whole time (in the background) and she never wipes them off).
