Plot hole: In the plane scene where Deguerin is holding a gun to the pilot's head telling him to come around and hit Krueger with the plane, it is pretty evident that the pilot's not in on the scheme, so why wouldn't he tell what happened as soon as he landed, giving away that Deguerin was the bad guy (or at least one of them)? Audio problem: At the beginning of the film when Kruger rescues the mafia guy and his wife they hurriedly get in the car. Kruger somehow manages to pull down the gear shift immediately before cranking the car up. Listen closely you can hear the ignition starting after he's pulled the gear down.

Factual error: All the containers have sheet metal covering the bottom, not visible bottom crossbeams as they should.

Visible crew/equipment: In the car after the bar scene, as Johnny gets the info from Kruger, the shadows of the camera rigging plays across the actor's faces.

Visible crew/equipment: As Kruger approaches the bar, the shadow of the camera shade plays on the back of the man's head just before you see who Kruger came to see.

Other mistake: After Kruger tosses the parachute, there is a quick close up, and it looks like the chute just got tossed from another plane. Also, half the time, the chute has dust or smoke coming from it, and sometimes not.

Other mistake: After Kruger throws the chair into the engine causing it to explode, the co-pilot starts to hit switches overhead. As he does, the panel shakes, like it is a poorly constructed set. If it were real, the craft would be grounded until the defect was corrected.

Audio problem: As Lee walks into the Cyrez building, the camera pans up to the top of the building, where a helicopter is landing. The problem is that it is an Apache attack chopper that is seen landing, but the sound is of an OH-58, a smaller chopper.

Continuity mistake: As Kruger puts the dead gangsters on the lawn, he pulls out a bag filled with fake blood. A second later, it suddenly changes to two photos of the victims.

Continuity mistake: In the beginning of the movie when John Kruger goes to Lee's house with the balloons; Darryl gets shot and John and Lee leave the house, get in the van and drive away. One of the guys outside shoots at the van twice, shattering the back two windows and putting two bullet holes in the windshield. In the next shot of John and Lee through the windshield, there are no bullet holes. The shot after that when they drive away, the shattered windows are not shattered anymore - you can see the reflection of light on the windows as they drive off.

Continuity mistake: During the scene near the beginning where Arnie sends the abandoned car into the river, have a look at the front passenger window. It is closed when they all get out of the car but is then fully open as the car hits the water. (00:07:40)

Continuity mistake: When John Kruger and the woman are discovered in the Cyrez office using the computer, John grabs her with both arms and throws her to the ground. In the next shot, with them still falling on the ground, John now magically has his gun in his hand to start shooting, which isn't possible in the time difference from the previous shot, and the fact he was holding her with two hands. Continuity mistake: In Chief Beller's office when Kruger is handed Cullen's file, Kruger sits down while holding his left hand in the middle of the file. When it cuts his hand has moved out to the corner of the file. (00:11:25)

Continuity mistake: When Kruger is figuring out which one of the three ID's that are fake, in the close-up shot you can see two different scales and a photograph on the table next to the ID's. When it cuts they are gone. (00:08:45)

Continuity mistake: You can see twice when bad guys are shot when the tape is cut and edited. The first one is when Arnold shoots the bad guy with the shot gun when looking for the blonde girl in the house, you can see just before he hits the picture on the wall that the tape is cut and replaced with the real guy instead of the dummy that was hit by the bullet. The second is at the end at the dock when Arnold has two of those really cool guns. He shoots a vehicle full of guys coming towards him and the vehicle blows up and flies over his head, and just before the vehicle blows up, you can see the cut & edit and then the car blows up and flies through the air. Undoubtedly replaced with a toy car that was blown up! (00:44:35)

Plot hole: Kruger's boss (James Caan) is in league with the bad guys. He convinces Kruger to help him protect the witness at the log cabin all in an effort to draw out Leigh from hiding and there are five bad guys at the cabin waiting for them. This of course is all a setup but what if one of those bad guys at the cabin had killed Kruger? Then Kruger's boss wouldn't know where to find Leigh and they would all be back to square one.

Suggested correction: A "What if?" scenario that doesn't actually play out can't be considered a plot hole. DeGuerrin is counting on the skills of Kruger and his own team being much higher than a bunch of cut-rate mercenaries, and that is exactly what happens. DeGuerrin enacts a plan that carries a high level of risk, but he and his cohorts have been backed into a corner. If they don't try, they will all go to jail. In addition, when you think about it every police/military raid carries the risk that someone you don't want to die might actually be killed. Risk is part of the job but that doesn't stop people from doing what needs to be done when the time calls for it, and to trust that their skills and training will keep their team safe.

Plot hole: The Feds had plenty of evidence to move in on Cyrez after Leigh's encounter with Donahue especially after his admission on camera, so there's no reason the company would still be in operation as if nothing had happened. At the very least all operations would be on hold until the hearings were concluded.

Suggested correction: Donahue didn't actually "confess" to anything specific, just some very vague things that the FBI wouldn't risk their entire investigation over.

Plot hole: The whole idea of copying the files to a disc (and a copy) is a pointless plot device. First of all since the information on the discs cannot be accessed outside of Cyrez the bad guys are putting an awful lot of effort into getting rid of it when all they need to do is kill Leigh. Second, if one has to be at the vault or Donahue's office to access the information then then it's no different than accessing the information on any given day of the week. All the Feds needed was a warrant to look at Donahue's files and everything in the movie could have been avoided.

Plot hole: Considering how much the bad guys stand to lose from Leigh's cooperation with the Feds they would have already been at her home ready to kill her long before her or her ex got there, and wouldn't need to wait for her to access the copy she made before swinging into action.

Suggested correction: They had already intercepted whatever evidence Leigh had because of their inside man at the evidence depot. Donahue was to be made an easy scapegoat since he admitted his guilt on tape and subsequently killed himself. With the scapegoat dead and the evidence lost the case would go away on its own. Killing Leigh at this point only makes the company look more guilty and prove that Donahue was not solely responsible.
