Continuity: When Selene slams on the brakes sending Lucian off the car, his arm blade disappears during his tumble, and reappears in the same shot once he stops. Viewed best in frame-by-frame.

Underworld (2003) - 39 mistakes
Directed by Len Wiseman, starring Bill Nighy, Erwin Leder, Kate Beckinsale, Michael Sheen, Scott Speedman, Shane Brolly, Sophia Myles (add more)
Genres: Action, Fantasy, Horror, Thriller
Yes, everyone knows that vampires conventionally don't have reflections, but almost every vampire film adapts/alters the legend to fit the story they want to tell (after all, originally vampires could go out in daylight), so them having reflections in this film isn't a mistake.
Factual error: Every time the silver nitrate is shown on clothing, it is shown as silver. Silver nitrate in fact is black once exposed to light.
Visible crew/equipment: During the final battle, a vampire shoots into a totally dark room and kills a werewolf. He then lifts his gun and people are seen reflected in the sights.
Continuity: At the end when the 3 vampires are shooting at Michael, Selene drops behind them. The blood on the left side of her head disappears and reappears a couple of times. When she's taken them out it's back again. And it gets bigger and smaller all during the time it's there.
Continuity: When Selene breaks the glass and takes out the old book the amount of broken glass on the book differ between shots.
Revealing: When Selene notices the sword laying on the rocks, the sword moves up about two inches between shots, you can judge it by looking closely at the handle of the sword.
Audio problem: Near the beginning of the movie, while Selena is looking at the CCTV video from outside the subway station, Kraven enters the room and talks to her. Throughout their conversation, the sound quality of his voice changes as he walks away from where the mic is located (by Selena).
Revealing: When Selene is firing her guns in the floor making a circle of holes so that the floor collapse, you can see in some shots the angle of her guns are pointing all kinds of directions. Instead of making a perfect circle the holes would be spread around the floor and the bottom of the walls in the hallway.
Continuity: In the beginning when Rigel is about to jump off the balcony, some of his coat is hanging over the rail. When it cuts his coat is gone from the rail.
Continuity: When Michael grabs Adam's arm and Adam tells him to let go, Michael is holding Adam's arm close to his chest. When it cuts, Adam's arm has moved over to his shoulder.
Continuity: At the shooting range, Selene is shown firing a black USP without a compensator. It then cuts to an over the shoulder shot, and she is now firing a two-tone USP Match (which she supposedly receives from Kahn to test out a few seconds later). It then cuts to a frontal shot of her, and she is once again using the same black USP. She never stopped firing, so she couldn't have switched guns.
Continuity: When Selene writes "VIKTOR" on the mirror you can see the writing has changed in the next shot before she wipes it off.
Revealing: After Selene jumps off the building at the beginning and lands, there is a very large upwards pull (that stays in place until the camera cuts away) in her cape near her head that is in fact the drop wire running under her cape to the harness on her back (although the wire has been edited out).
Revealing: In the fight scene between Michael and Viktor, Viktor throws Michael in to a column and Michael lands face down. You can see his wire harness bulging under his pants.
Revealing: In the fight scene between Michael and Viktor, you can see several times where Michael's dark colored make-up has been wiped off his lips.
Continuity: In the first scene when Rigel is crouching, taking pictures of the werewolves, he is holding the camera in his right hand just as he is about to jump down. However, when he is jumping, the camera angle changes and it can be seen that he is no longer holding a camera in his right hand.
Continuity: When Lucian goes flying over Selene's car, in the overhead shot we see that the road below him has large parts with visible concrete because there are no paving stones. But in the next shot, a close-up of Lucian landing on his feet, the road is fully paved with paving stones.
Continuity: As Viktor's tomb is opening there is a shot from above showing Selene looking down the opening tomb. Watch the placement of her feet. In the next shot, her feet have moved closer to the opening.
Continuity: When Selene is chasing the Lycan in the subway and is surprised by him firing at her from behind her she turns around and gets down on her knees and fires back. When it cuts she gets down on her knees again.
Continuity: When Selene discovers the blood on the wall in the subway chase the blood changes appearance between the shots.
Revealing: In the end fight scene between Viktor and the hybrid Michael, Viktor picks Michael up and throws him into a stone column. Terrible wirework then comes into play: Michael travels far too slowly towards the column for someone who was just thrown, he makes one perfect spin in the air on his way, and when he finally connects with the column, it's a delicate bounce off before the wire drops him on the ground. His legs don't even move from the "impact".
Revealing: When Lucian is draining blood from Michael, the tip of the syringe bends like rubber.
Continuity: Selene is in Michael's apartment looking through photos, you can see the door to the apartment fully open in the background. When the phone rings, and we see Michael walking towards his apartment, the door is only partially open.
Factual error: Selene's guns are apparently capable of firing 30 rounds without reloading. Considering the size of the ammunition, this is physically impossible.
Continuity: When Selene runs up the stairs to release Michael from the chains, the door she left wide open is almost closed when the pursuers reach it.
Continuity: After Selene has killed Viktor she approaches Michael from behind. We see him turning his head. When it cuts he turns his head again.
Other: When Selene's car plunges into the Danube, her car hit the bottom of the river on it's side (the passenger's side). When the camera closes up to the window of the car, you can see water flowing in from the roof of the car (as a result from the holes created by Lucian). It should flow (or at least accumulate) towards Michael's direction because of gravity but it did not.
Continuity: When Michael was captured and hung up, and meets Lucian face to face, Lucian injects blood into him and the syringe has an air bubble in it. In the next shot, the bubble has gone and the syringe is entirely filled with blood when being injected into Michael.
Continuity: During the fight scene between the two Lycans, it shows a shot of Taylor (short Lycan) receiving a cut across the left side of his chest, but when it shows him in his human form it is on his right.
Revealing: After Selene bites Michael, Viktor picks him up and throws him through a wall. On the other side, he lands in a pool of water as do lumps of the wall, which float briefly before the shot ends.
Continuity: When Erika discovers that Michael has been bitten - just before he leaps out of the window and escapes the vampires' mansion - there is a patch of blood above Michael's eye that disappears.
Continuity: In the close-up shot of Selene biting Michael, she only has fangs on her upper canines whereas in the rest of the film she has fangs on both her upper canines and upper lateral incisors.
Visible crew/equipment: When Michael asks Selene who started the war she answers "They did. Or at least that's what we are led to believe". If you look really close on the side of the window you can see the shadow of the boom mike move.
Continuity: There is a shot of Kraven standing by a window, looking out into the night after Selene has run off. Halfway down the window, there is a reflection of a face - it isn't Kraven's. It looks like a woman watching him, perhaps Erika? But there is no indication that there is supposed to be anyone else in the room.
Continuity: In the last fighting scene between Victor and Michael, Victor's face gets scratched and it starts bleeding. In the next shot, his face is completely cleared of his scratches. And in the next, the scratches appear again.
Revealing: At the end of the film when Viktor and Michael are fighting you can see Viktor pulling his punches.
Continuity: After Michael has been shot with the liquid silver bullets, his shirt and the seat of his pants are stained with the silver residue. When Viktor throws him into the water and he turns over during the transformation the silver is gone from the front of his shirt. In the fight between them, the stain is also gone from his pants.
Revealing: The edges of Selene's bright blue contact lenses are VERY visible in the scene near the end where she is on the ground, talking to Viktor.
Audio problem: At the beginning, when Selene is hunting the Lycan though the subway tunnels, she is pressed against a wall, and she starts to look around the corner. Then suddenly a subway train passes by. Shouldn't we have been able to hear it?
