Continuity: When Spartan and Phoenix are fighting in the sewers, Edgar Friendly shoots his gun at Phoenix and you see the catwalk that Phoenix was on tip to its side and Phoenix dropping down to the ground with the plank of the catwalk right below him. When Phoenix lands on the ground the plank is nowhere to be seen. It should have been right below him.

Continuity: When Spartan and Huxley arrive at the cryogenics place to battle Phoenix, Phoenix sees them on the monitor walking into the building and you see Spartan on the left and Huxley on the right. The camera angle changes to an inside view and Spartan is now on the right.
Continuity: When Spartan drives the car through the glass window, you see glass everywhere. The camera angle changes to a wide shot and you see the car driving away but no glass on the ground.
Continuity: When Spartan slams the Cryogenic pill on the ground he is about 3 feet in front of Phoenix, but when you see him jump up to grab the claw, he is seen much farther away.
Continuity: When Spartan switches seats with Huxley in the police car, the outside shot shows him get into the back seat. When the shot switches to Huxley in the driver's seat, we can see the back and Spartan isn't seen anywhere in the car.
Visible crew/equipment: When Pheonix falls through the glass at the museum into the LA exhibit, you can see him sliding down a clearly visible safety rope. His feet are even in the position to be sliding down the rope.
Continuity: The guard Phoenix throws through the glass case has on footwear which resembles socks. As he is smashing through the glass, he has on heavier leather boots.
Continuity: After Spartan goes through the glass SAPD sign, the car flips over on its roof. But when they show the car up close its now right side up.
Continuity: During the final fight scene, Spartan grabs Phoenix and they land on top of the ice block and roll off in front of a few metal jugs. The camera angle changes to a close up and the jugs are not there anymore as you see Spartan punching Phoenix.
Factual error: The display on John Spartan's cryogenic reads, "August 3, 2032," which will fall on a Tuesday. Later that same day, Huxley says to Warden Smithers, "It's a beautiful Monday morning"
Continuity: When Spartan is hunting for Phoenix at the Cryogenics lab, the 2 scratches on his face appear and disappear throughout the scene.
Continuity: During the glass floor shootout at the museum Spartan shoots a light that falls through the glass. Right after Spartan shoots the light it goes dark but the close up of the light falling and hitting the floor shows the light to be still lit.
Continuity: When Spartan is initially frozen in the cyro-liquid, air bubbles and ripples are in the ice around his mouth. Any following shot of him in the ice block shows no bubbles or ripples.
Audio problem: In the European version, Taco Bell is replaced by Pizza Hut, however, when Huxley says to John Spartan that they're going to Pizza Hut, her lips say "Taco Bell".
Continuity: When Phoenix is fighting the police at the kiosk, he knocks the sunglasses off of one officer, but in the next shot the glasses are back when the cop is knocked out.
Continuity: In the opening scene as Spartan is getting ready to bungee down to the roof, he kicks a bag out of the chopper that pulls the cord out with it; the bag is never seen in any other shots of the cord as Spartan drops.
Revealing: Watch closely as Phoenix is about to fight the cops near the computer kiosk: the same two people wearing white robes pass behind the officer with the hand-held computer twice.
Continuity: When the Cryo-Prison worker opens the cabinet that holds the cylinders that contain the blue "freezing balls" to freeze Spartan's slush, the slot on the center left is empty. When the shot changes to the worker pulling the cylinder out, the slot to the lower left is now the empty one.
Continuity: During the Oldsmobile dealership scene two robed citizens are shown to be looking at small scale model cars on the rear wall (blue car) and side wall (red car) in the showroom. When Huxley and Spartan bust through the floor in the elevator the two citizens are missing.
Audio problem: After John Spartan crashes through the police station sign he says, "All I need is a needle and a thread." But when the camera is behind him and he says the line, "I really didn't say that, did I?" his mouth doesn't move when his face is in frame.
Continuity: In the scene where Huxley, Spartan and Garcia get ready to go into the sewer system to find Friendly, both car doors to the police car are open. Yet when Phoenix escapes from the sewer system and approaches the cop car to steal it, the passenger side door is closed.
Continuity: When Spartan is frozen, his mouth is open, but when you see him in the block of ice his mouth is closed.
Continuity: During the first fight scene between Phoenix and Spartan, Phoenix sticks his knife into several barrels of fuel and you see the fuel all over his knife but when he lays it on the table it's dry.
Continuity: When Phoenix throws Spartan over the bonnet of the red car in the museum, Spartan lands in a puddle that soaks his hair. In the next few shots, the hair keeps changing from being soaked to being dry.
Continuity: During the scene where Phoenix is questioning the leader of the future society in his office, he says at least one word (hell) that he should have been fined for, and isn't. We see that there is a ticket printer in the room in a later scene, when Savage sets it off during his own confrontation.
Continuity: When Phoenix blows up the door sealing the Armory, it is just about the size of a normal door, but when he runs through it to escape into the main hall, the door is four times larger.
Factual error: During the fight in the museum, Spartan gets three shots out of a shotgun which only holds two shells.
Continuity: When John Spartan is frozen in the cryochamber, at the beginning of the movie, if you watch during the credits - while the time is going by the small circular monitor that is on his chest, it changes from the left side to the right side of his chest in almost every shot.
Continuity: When they are in the Cryogenics lab, the tear and blood spot on Spartan's shirt changes throughout the scene.
Continuity: Several times during the movie, Simon Phoenix's shoulder pads (one side resembles an ATV tire) switch sides.
Plot hole: When Spartan jumps through the window in the excavation scene of the museum, he crashes into a room with bottles of drink on the shelves. Surely this room too would have been filled in and the window smashed during the earthquake, and the museum wouldn’t have bothered to excavate it as it can't be seen from above due to the blanked out window. So what is that room doing there?
Visible crew/equipment: This is during the firefight in the makeshift restaurant down in the sewer. When John Spartan grabs the table to use as a shield, you can see the cable going with the table that is used to set off the sparks for the bullet contacts.
Continuity: When Simon Phoenix gets out of the sewer system (after gunning it out with the rebels and John Spartan) his left eye has the brown contact and his right eye has the blue one after going the entire movie with those contacts in the opposite eyes.
Continuity: When Phoenix first gets out he makes his way to a computer and the police come to arrest him. Phoenix takes one of the policeman and pushes his head through the side glass pane and walks away to fight the other officers, but when Phoenix goes back over to the computer the glass is not broken.
Factual error: Simon Phoenix uses the warden's removed eye to escape from the cryoprison. The eye is on a spike, and since the retina includes the back portion of the eye, the spike would have not only destroyed the shape of the retina, but blocked the scanner from reading the complete retina as well. There's no way the retinal scanner would have let him pass.
Continuity: When Spartan and Huxley go to Raymond Cockto's place, Spartan shoots all of the monitors with a silver gun, yet when Spartan points the gun at Cockto's head, which we see on the surviving monitor, it's a black gun.
