In the scene of Sam and Frodo on the slopes of Mount Doom, the entire scene has been flipped. Peter Jackson wanted the Hobbits to travel from left to right across the screen for all three movies, so the editors had to flip this scene to achieve that. This is also why any cuts/dirt smudges on their faces seem to change sides. Only when Frodo and Sam return to the Shire and go to the Grey Havens, do we see them traveling right to left. See more...

Mistake Continuity: When Pippin approaches sleeping Gandalf there is a shot of Gandalf hugging the cloth covered Palantir, yet when Pippin waves his hand in front of Gandalf's eyes the cloth is wrapped around the Palantir differently. When Pip places it on the blankets and opens the grey cloth, in the first close-up, then a wide shot and subsequent close-ups, the cloth lying around the palantir is arranged differently.

Mistake Continuity: The braids over Legolas' ears on his scalp, change in position, shape, and how it is braided, in consecutive scenes or shots, as well as the amount of his side hair that is swept up into the back braid. Some examples are, as he and Gimli ride Arod on the Paths of the Dead, Legolas says, "...and Isildur cursed them," and then after Aragorn says, "I do not fear death," before he runs into the Dimholt Gate. Also later in two consecutive shots, when Legolas says, "And what about a friend," and then when they battle at the Black Gate.

Mistake Continuity: In the scene at the beginning of the siege of Minas Tirith, as Denethor looks down, when the Troll is heaving the first rock onto the catapult, the first shot shows the rock being flat with a flat bottom. In the next shot, once on the catapult, the rock is taller.

Mistake Continuity: When King Theoden is lying under his horse, when Eowyn starts to battle the Witch King, he has spots of mud on his right cheek that are very clear. After Eowyn has slain the Witch King the spots on Theoden's face are now faint smudges. After he dies and she leans down to hug him, the spots are now as they appeared in the first shot.

Mistake Continuity: In the scene where Aragorn suggests attacking the Black Gate in order to draw out the orcs and give Frodo safe passage, the amount of Gandalf's hair hanging loose from his pony tail varies from shot to shot.

Mistake Continuity: Aragorn is summoned to Theoden's tent, where he is greeted by Elrond, who removes the hood from his head. Just before Elrond steps towards Aragorn, in the close-ups, the hood at his shoulders changes position within these shots.

Mistake Revealing: At the end of the movie when the hobbits leave the Shire, there's a shot of a lake with trees in the background. The reflections of the trees don't match their actual shape.

Mistake Continuity: When Sam rescues Frodo in the Tower of Cirith Ungol and gives him back the Ring, Frodo puts it around his neck and you can see it against his bare chest. The chain is noticeably much longer and thicker than the one he's been wearing previously.

Mistake Continuity: During Aragorn's speech to his army at the black gates, the direction the flags on the spears blow constantly changes. When we see him from behind facing them, they blow to his right, and when we see him in the POV from within the crowd, they are blowing to his left.

Mistake Continuity: In one of the fighting scenes Legolas is ready for battle, but when you look at him first his hair is behind his shoulders the next camera shot showed his hair perfectly laid out in front of his shoulders, then the next shot his hair is back behind his shoulders again.

Mistake Continuity: Gandalf discusses Pippin's contact with Sauron to the others, in the Golden Hall of Edoras. Right after Gandalf's line, "They will be," in the shot of Gandalf's back, all of his hair is in underneath and in front of his cloak, but when Gandalf approaches Aragorn to say, "You must come to Minas Tirith..." his hair is over the top of his cloak.

Mistake Deliberate "mistake": At the end of the film in the shot from behind as Bilbo is walking towards Elrond and then up the boat ramp, we see the head of the "little person" playing the small version of him. His head is actually in a different proportion compared to the previous shots. The top of his head is considerably larger than before.

Mistake Continuity: When Aragorn, Legolas, Gandalf, Gimli etc, ride up to the gates of Mordor, the main characters go up to the gate on their own to demand it opens. The trails the horses leave on the way towards the gate are different to those that you see in the shot when they retreat after the gate has opened.
