Revealing: In the scene inside Mount Doom after Gollum bites Frodo's finger off and it appears they have both fallen into the lava, Sam looks over the edge of the cliff and sees Frodo hanging on. Just after Sam says, "Take my hand!" as Frodo tries to grab Sam's hand, you can plainly see Frodo's "missing" finger bent in half to give the appearance that it is gone, complete with the blood and gore seen just after the finger was bitten off initially.

Continuity: When Eowyn and Merry are talking while they are resting on the ride to Gondor, Eowyn's hair goes from tucked into her shirt to a few pieces untucked and then back to all her hair tucked in. (Extended Edition).
Continuity: After getting caught in the web at Shelob's Lair in Cirith Ungol, Frodo's Elven cloak and the rest of his clothes are covered in web material. Following this, there are quite a few consecutive shots in which the amount, position, or existence of web on his body, changes within the close-ups and wide shots.
Continuity: Frodo falls at Shelob's Lair, and he lifts his head to see Galadriel, as she reaches her hand out to him, In the first close-up of Frodo he has a green leaf in his hair, in the second close-up, it's a brown leaf and in the third close-up it's the green leaf again. There are also differences with other leaves around him.
Continuity: When the Witch King attacks Theoden and Eowyn fights him, the Witch King's helmet changes between shots. The gap at the bottom where the chin should be varies in width. In some shots the gap is small, nearly non-existent, while in other shots the gap is rather large.
Continuity: While Eowyn is with dying Theoden, after he says, "my body is broken" there is a black smudge over Eowyn's right eye in the next three shots, whereas it is not in the previous or following shots.
Revealing: When Merry and Pippin are entertaining the men and women of Rohan with their "Green Dragon" song, there is a woman at the right rear corner of the table on which the hobbits are dancing who looks at an empty spot behind Merry at the finale. No doubt the actress was looking at the spot she presumed the hobbit would be "pasted" into during editing, but she's off a few feet. This is further emphasized when she hold out both hands in a "take a look at this" gesture and again indicates the empty space behind Merry.
Continuity: When Éowyn whips off her helmet and stabs the Witch King, her hair is neatly parted, long and straight. Yet, as she crawls in pain, away from Gothmog, her hair is in nothing less than a total mess. Then when Éowyn, still injured, crawls to Theoden her hair is once again nice and tidy. (Extended Edition)
Continuity: At Edoras, after Gandalf throws the blanket over the palantir, Pippin's hair is on the floor above his head. However, in the next shot, his hair is completely under his head.
Continuity: After Arwen's vision, when an Elf escort says, "...we cannot delay.", in the close-up he faces her and his hair is behind his shoulders. In the next wide shot as Arwen begins to leave, the escort now turns to face her, his hair is in front of his shoulders and the neck/shoulder area of his cloak is different than the previous and following close-up. In the next close-up, his hair is behind his shoulders again.
Continuity: After falling near Frodo's body, Sam thrusts the Phial of Galadriel toward wounded Shelob as she recoils into her lair. In the shots behind Sam his hood is at his left shoulder, but in the shots from the front and in the overhead shot as he reaches Frodo's body, his hood is centered.
Continuity: During the scene between Denethor and Pippin in the Main Hall at Minas Tirith, Denethor is eating and has a red mark running down his chin. There is a shot from Pippin's perspective showing the right side of Denethor's chin with the mark. The next shot is from Denethor's left with Pippin in the background, and Denethor turns towards the camera and the mark is gone. The shot is then returned to Pippin's perspective, and the mark has returned for the rest of the scene.
Continuity: During the "It will be spring soon" speech between Sam and Frodo on the slope of Mount Doom, notice that the light hits their faces differently based on the shot. If it is a wide angle shot, the light is on the right side of their faces/bodies. However, if the camera is in tight for a close-up, the light is shining from the left side.
Revealing: At Dunharrow, Merry watches as everyone rides off, and in the shot of his back Éowyn's fingertips reach his right shoulder. In the next front close-up, she's still further back, note her fingers behind his head, but he starts to rise off the ground even as her hand is just coming closer.
Continuity: When the Battle of the Pelennor Fields is wrapping up, you see Aragorn with some of his hair plastered to his forehead with sweat. In the next shot (in the background you can see the Dead swarming up a mumak), his forehead has nothing on it.
Continuity: When Aragorn beheads the mouth of Sauron, his left cheek is splashed with a fair amount of black blood which drips downward. In the very next shot of Aragorn's face, the amount of blood has been reduced to a few dried spots.
Continuity: After Pippin retrieves the Palantir, in the close-up facing him, he stands directly in front of the center of the spokes at the wheel's hub, as he hands the Palantir to Gandalf. However in the next shot as Gandalf takes hold of the Palantir, they are quite far from the wheel. Next shot Pippin stands close again. (Extended Edition)
Continuity: Deagol stands up on shore, raises up his right fist and in the wide shot there are blades of grass on his fingers. In the next close-ups while there are no grass blades, the tips of his fingers are visibly much cleaner than in the next wide shot as Deagol stares at the Ring in his palm with extremely muddy fingers. Yet in the next close-up of Deagol's fingers closing over the Ring, after Smeagol says, "Give us that, Deagol, my love," the fingers look the same as in the previous close-up.
Continuity: When Shelob retreats into her lair, Sam runs to Frodo and lays Sting down on the ground. It lies on dirt that has white rocks showing through underneath, which is even seen in the last overhead wide shot of Sam holding Frodo. Yet in the next close-up of Sting glowing blue, it is lying on plain dirt.
Continuity: When Frodo, Sam and Gollum reach the "dead city" of Minas Morgul, the stones, pebbles and dirt along the path and on the ground, below the Gargoyles, change in the consecutive shots.
