Chucky: Why fight it, Andy? We're going to be very close. In fact, we're gonna be fucking inseparable.

Directed by John Lafia, starring Alex Vincent, Brad Dourif (add more)
Continuity: When Watson opens the door with his card, you see him holding the key in his right hand while opening the door. The way he holds it changes in the next shot.
Revealing: When Mattson pulls the card through the card reader in order to open the door, there is no lock mechanism on the door at all when we see him opening the door and entering the room.
Continuity: Between shots from when the worker is tapping the buttons, to the closeup when the eyes are put into Chucky's sockets, the jars, boxes, pencils and other items on the desk around the Chucky doll move and change their position.
Continuity: Near the beginning, the Chucky doll causes a worker to be electrocuted and fly backwards. As he somersaults through the air, and through the window, it is clear that in this angle he will land on his front. But in the next shot we get a close-up of him lying on his back.
Continuity: After the worker has been electrocuted, Mattson and his boss walk towards the exit. We see Chucky sitting by the exit door (rigid and lifeless) and his right arm is down on the floor. In the next shot, his arm is in his lap.
Continuity: When Joanne and Phil talk to Grace Pool about fostering Andy the pot of pens on Grace's desk move about between shots.
Continuity: When Joanne, Phil and Andy are in the car after they have fostered him, the shots alternate from the backseat to the front, during the scene. During the shot changes Joanne's purple jumper sleeve keeps differing in how far it is rolled up.
Continuity: After fostering Andy, as they drive along talking about what Andy likes to eat, in one shot Phil turns the steering wheel, and the car starts to turn a bend, but in the next shot they are driving straight, down a road.
Continuity: When Andy gets a photo of his mum and him out of his pocket, the way he holds it differs between shots.
Continuity: When Andy, Phil and Joanne are driving, a big truck suddenly appears in front of their car so Phil has to urgently stop the car. Then you see the car has pretty much stopped. It then cuts from under the truck and you can see the car is still moving a few feet before stopping.
Continuity: When Phil stops the car due to nearly hitting a truck, the truck turns and drives along beside the car. The truck starts passing the car, but then as the shot changes, you can see out of the car window the same part of the truck passing the car once again.
Visible crew/equipment: When Andy goes upstairs after being introduced into the Simpson's home, he walks down the hallway and then backtracks to Kyle's bedroom door and opens it. As the shot pans closer in as he opens the door, the camera and its operator are shadowed on the door.
Continuity: When Ms. Simpson and Andy enter Kyle's room, Kyle's lighter is lying on the table beside the cloth. When they leave, the light has moved onto the cloth.
Visible crew/equipment: When Andy goes into the closet and he knocks the skateboard, and Chucky falls down, you can see a wire attached behind the skateboard so it doesn't fall down on the actor.
Continuity: When Joanne is showing Andy his new room, he opens the closet and Chucky falls down, the doll has its head facing the room. In the next shot, Chucky is in a different position with his head facing the closet.
Continuity: Chucky's eyes are made so that they blink and move. However, in the scene where the business man is stuffing Chucky into the trunk of his car, Chucky's face is different because his eyes are drawn onto his face. The same thing happens with his mouth throughout the movie.
Continuity: Near the beginning, Mattson tosses Chucky into the back seat of the car head-first. In the next shot, Chucky is turned around, as if he was chucked in feet-first.
Audio problem: When Mattson throws Chucky into the back of the trunk, he tries to close the trunk. However, the trunk won't close because it hits Chucky. You can barely hear any sound when the trunk bumps into Chucky. When it cuts to Mattson trying again, you can hear a bumping sound.
Continuity: The toys in the back seat of Mattson's car change from when Chucky is tossed in, to when Chucky comes alive.
