Other: When the ice cold assassin (played by James Woods) is slaughtering all of his erstwhile employer's killers for hire, he shoots one of them dead on a bed. Obviously, the camera operator hadn't read his script (or maybe drank too much coffee that day) but when the shot goes off, he gets a huge fright and the camera frame jerks visibly.
Other: Near the end of the movie, in the mansion overlooking the water, James Woods is eliminating the bodyguards. As he shoots the one at the bathroom sink, blood sprays all over the mirror, but the bullet never breaks the glass.
