Who plays the man who rides in the elevator with Jackson when they go to Creek Mountain Facility? [I am assuming you are refering to the man in the elevator who also goes with them on the mission? If so, it is John Diehl. Most recognized from "Stripes", "The Client", and he played a recurring character as a detective on the TV show "Miami Vice".]

Did they give a reason why O'Neill was recalled? Did he have special qualifications that made him the only one capable of carrying out the mission? [He was known to be suicidal. He was picked because they wanted someone who wouldn't mind getting killed as the gate was blown up on the other side.]

What does Dr. Jackson say when they send the probe through? Is it, "Where are we on that map?" or, "Where will it be on that map?" [It's "Where are we on that map?" the reply given is "The blue dot". He then traces the path to the other side of the map where the probe is shown, which is on the other side of the universe.]
