| 1 | Coronation Street | In a recent episode (April 08), Lloyd referred to Steve as Simon during a conversation in the Rovers. Simon happens to be Steve's real name. Oops. |
| 2 | The West Wing | Sam says the "state-of-the-art" oil tanker he recommends can carry 2.2 million gallons of oil and weighs 308,000 tons. The Exxon Valdez could carry over 60 million gallons (1.48 million barrels) and weighed only 211,500 tons. The writers mistakenly swapped 'gallons' for 'barrels' so that this enormous tanker carries very little oil. It would actually carry 2.2 million "barrels", which equals 92 million gallons. |
| 3 | Lost | Desmond claims that he was saving Charlie's life by saving Claire from drowning by jumping in before he does. However, if Desmond had not had a flash to witness the event of him dying, Charlie wouldn't have known it was happening either as he followed Desmond to the event. As we find out later, they were a mile inland, so there's no way Charlie could have found out until a long time after Claire had drowned. |
| 4 | Buffy The Vampire Slayer | When Vamp Willow is locked in the book cage, she asks Cordelia to let her out. Since it is established that vampires possess greater strength than humans, she could have easily yanked the door off the hinges. <em>[While the cage is strong enough to contain Oz during the full moon, he has managed to escape it before, as evidenced by the fact Giles keeps a tranq gun in the library as a contingency. (Not only that, but Willow and Faith have both been required to shoot him with it.) Since Oz has managed to escape before, it's not unreasonable to think it should be a piece of cake for Willow.]</em>. |

| 6 | Life on Mars | When Sam and Gene enter the pub with the T.V., they both stand at the bar with Gene leaning one hand against the bar. In the next shot Gene is leaning on it with both hands. When the shot cuts again he is back in his original position with one hand on the bar. |
| 7 | 24 | Not only does Edgar knows Chloe's high-level access code - he says it aloud to Spencer. And to top it all off, you can read this code on the screen - without any asterisks . |
| 8 | My Family | Just as Ben starts to sing about life, there is a shot of a picture on the piano. If you look at the picture, not Ben, you will see the reflection of a camera or microphone arm. |
| 9 | Hawaii Five-0 | When the bad guys pull up to the loading dock warehouse, the door of their shiny black sedan reflects several large round studio lights. |
| 10 | Winds of War | Part 7: The car Captain Henry parks near the cliff at the very end moves itself between takes. It's sitting on the road when he parks it, but when we cut to an overhead shot after he gets out, the car has suddenly moved off the road and into the dirt alongside it. |
| 11 | The Tudors | In episode 18, as Henry is lying on the table after the jousting accident, the camera pans up and reveals a fluer de lis on Henry's breastplate. The fluer de lis is/was the symbol of the French King and it is highly unlikely that it would be so prominently displayed on the English king's armor. |
| 12 | Full House | Jesse is first introduced as Jesse Cochran but his last name later changes to Katsopolis in season 2. (This was known by the producers). |
| 13 | Star Trek: Voyager | Just after Voyager is hurled across the galaxy, Captain Janeway and the crew are recovering from the impact. Janeway's hair, done in an updo bun, has become disheveled and strands are hanging loosely. Briefly after, when she's in engineering, her hair is now neatly coiffed again; the hairstyle is a little too complicated for her to have quickly pinned the strands back so perfectly . |
| 14 | Star Wars: Clone Wars | All the Vulture Droids in this series are brown like in the first prequel film. But being the Clone Wars they should be blue with markings of the Confederate Navy. |
| 15 | Stargate SG-1 | When the hallucinated Jonas is leading Dr. Kieran upstairs, a red light can be seen glowing on his hip where his Beretta is holstered. It looks as if Corin Nemec is carrying an Intar (a training weapon with a glowing red crystal in the grip) prop instead of a normal pistol. |
| 16 | Ashes to Ashes | The Audi Quattro used throughout the show has single unit rectangle type headlights. These type of headlamps were not used until the 1983 model year for that car. The earlier type were of four smaller separate square headlights. |
| 17 | South Park | When the British knight beheads an executive in the office and gets shot through the glass the sounds of these three things aren't there. |
| 18 | Time Trax | At least three different birth years are given for Darien Lambert in various episodes, among them 2163, 2160, and 2157. |
| 19 | Terminator: The Sarah Connor Chronicles | At the end of the first episode, just before Sarah, John and Cameron travel to the future, Sarah blasts Cromartie and you can see his head fly off. In a future episode, Cameron reveals that Cromartie has traveled to the future along with them by pointing out his bare endoskeleton head in the background of the video that was taken of them just after they arrive in the future. It is supposed to be impossible for anything non-biological to time travel, so why is it that Cromartie's head made it through even though it was missing the surrounding living tissue. |
| 20 | Monk | Look at the stuntman who stands in for Tony Shalhoub when the balloon crashes and the basket scrapes and bumps along the ground. He is about fifty pounds lighter than Shalhoub, looks nothing at all like him and his hair looks like a Harpo Marx fright wig dyed black. He is one of the most obvious stand-ins I have ever seen. |
| 21 | Scrubs | When J.D. comes out the bedroom and finds Turk and Carla waiting, Turk's hand goes from cupping his chin to resting on his fist, depending on whether the shot is from the front or from the back. |
| 22 | Beverly Hills, 90210 | Near the beginning of this episode Andrea is awakened at 3 A.M. by an alarm clock to take a pill to stop her preterm labor. Several times in the episode it is mentioned she must take the pill every 4 hours. Later the alarm goes off at 8 PM the same day and she says "time for the pill again that is not doing me any good". If the alarm went off at 3, 8pm doesn't fit the 4 hour timeframe. |
| 23 | My Friends Tigger & Pooh | The letters and symbols on the characters' reflections in the mirror look like their regular counterparts, when they're supposed to be in reverse. |
| 24 | Prehistoric Park | When Theo hits the fence it becomes snapped a bit but in the next shot you can see him preparing to charge at the fence again, and it is whole again. |
| 25 | Gilmore Girls | When Kirk chides Ms. Patty in Doose's Market for sampling food from the produce section, he is unfamiliar with who she is. But in subsequent episodes, it is established that Patty and Kirk have known each other since he was a child. |
| 26 | Charmed | Paige is sent back to when Grams was her age; this shows that they are family and that they share similar traits. Grams having orange hair is a mistake because Paige's hair is not naturally orange, therefore has nothing to do with genetics. |
