| 1 | Smallville | Lois digs up Chloe's grave site to unearth her casket. It is obvious that the casket is not buried six feet deep. You would think, will all that is going on, that LuthorCorp would have at least buried the casket deeper to deter inquisitive eyes and maybe throw in a few scattered bones for good measure. |
| 2 | Charmed | The newspaper which you can see Prue holding at the end of this episode (right before she 'half-blinks' to shut the door) has something written on it like yadda yadda yadda 'Doubt Fire' yadda yadda yadda. Yep, thats right, the same newspaper from Mrs Doubtfire. |
| 3 | Angel | In the last scene of the episode, Angel kicks the guy out of the glass building. This guy's a vampire, and you can see his reflection in the glass windows. Vampires aren't supposed to have reflections. |
| 4 | Days of Our Lives | When Billie visited Georgia's grave in Salem, she acted as if the baby was buried there, yet, Billie should know that the coffin buried is just a bunch of rocks and the real Georgia is buried where Earl, Wayne, and their mother live by the bayou. Billie lied to Bo about Hope being the cause of their baby's death and buried the rocks in the coffin to make it look like the baby had died after her fight with Hope, rather than before. |
| 5 | The Simpsons | When Marge is speaking on the phone to her sisters, her sisters say something that sounds like they said that Homer is ugly. Marge then puts down the phone wire that she was curling round her finger. It is easier to see this when you are using DVD, because then you can go into slow-motion, but you can temporarily see that the phone wire disappears when Marge puts it down. It is at the very end of an early scene, because as soon as this happens it changes to another scene. |

| 6 | Kim Possible | In the scene where Kim is running across grass-covered Japan to stop Duff Killigan, when she talks her lips don't move along to the words. |
| 7 | Gilmore Girls | Season 6, 9/20/05, "Fight Face" While in Luke's Diner, Luke hands Lorelei a cup of coffee in a 'to go' cup. When Luke puts the coffee in front of Lorelei, the cup makes a hollow sound when it hits the counter, obviously indicating that there's no actual coffee in the cup. |
| 8 | Friends | Chandler tries to open the door that leads to his room, but Joey has sawed the door in half so only the upper half opens, and Chandler then falls over the bottom half. He has to hit it with some force to fall over it, but it doesn't budge an inch. |
| 9 | The Super Mario Bros. Super Show! | On the "Mario's Greatest Movie Moments" DVD of this series, from the main menu, go into the trivia game. The fifth question asks you about the Super Mario Bros. Super Show episode 'Count Koopula.' It asks you "What's room number that the Mario Bros. were put into in Koopula's Castle?" It's supposed to say "What's THE room number that the Mario Bros. were put into in Koopula's Castle?" |
| 10 | Alias | For secret agents, Sydney and Dixon are not very dedicated to secret identities. At several points they refer to each other by their real names in front of other people while they are under aliases. Examples include The Confession party and The Coup Las Vegas scenes. |
| 11 | Full House | Jesse says Becky will blink once for a date and twice for no date. Becky blinks twice, and Jesse says "Break out the champagne" meaning they have a date, but didn't he just say two blinks mean no date? |
| 12 | The King of Queens | On outside shots the front door is on the extreme right. Shots from inside the house show the living room on the left and the door in a central position. |
| 13 | Lilo & Stitch: The Series | Pleakley reacts to Jumba's "Where is my experiment?" too early. |
| 14 | Mighty Morphin' Power Rangers | Episode 13-12: "Shadow (2)." When Cruger is challenged to defeat 100 robots, a red countdown meter appears on the screen, ticking down each time Cruger dispatches an opponent. When the counter is at 21, Cruger slices his sword at a robot, which then goes down. But the counter stays at 21 and only goes to 22 when he slices at the next robot. Every robot in the sequence goes down after a single hit from Cruger's sword. |
| 15 | Blackadder | The episode "Bells" and the episode "Head" appear to be inversed in their order - Lord Percy has a beard at the beginning of "Bells," and shaves it off. However, in "Head," he's got it back. In all of the other episodes, he has no beard. Also, the song at the end of "Head" mentions Lord Blackadder's origins (it mentions Edmund's great-grandfather), which makes it more a follow-on from the 1st series. |

| 17 | Everybody Loves Raymond | When Deborah wants to show Raymond's newspaper article to Frank, she calls him "Ray" instead of Frank. |
| 18 | Monk | In the flashback where Monk is explaining how the money was stolen, the principal finds the 5 dollar bill he wrote "good luck" on and holds it up for everyone to see. The bill he holds is a modern day 5 dollar bill which were introduced in the past few years, with the enlarged Lincoln. The flashback takes place in the 70s. |
| 19 | Seinfeld | When Mr. Bavalaqua is telling Jerry and Duncan the rules for the big race, there is a male extra right behind him who is mouthing Bavalaqua's lines as he's talking. |
| 20 | Family Guy | In the very new episode where Lois stops breastfeeding Stewie. When Stewie sneaks into his parents' room and attaches the breastfeeding pump to Lois, he places the cups too high on her chest for them to be on her nipples. You can see this, especially after he removes the cups. |
| 21 | The X-Files | In the episode "Musings of a Cigarette Smoking Man," when CSM is writing his letter of resignation on his typewriter, you can see that the letters are already there - he is simply typing over them. |
| 22 | Attila | In the party/orgy scene where Aetius is showing off Rome to Attila, one can see a woman with her back to us reveal herself to a man and woman. Despite her veil, one can see her thong underwear. This type of underwear is a 20th century innovation. |
| 23 | Phoenix Nights | At the start of the line dancing scene where Ray Von is deejaying, if you have the subtitles on, it says the song is Hillbilly Rock, Hillbilly Roll by the Woolpackers. It's actually 5,6,7,8 by Steps. (I'm quite embarrassed I know that). |
| 24 | Even Stevens | During the yard sale, in the first shot of Donnie's area where he has all his sports stuff, the sign behind him reads "Donny's Corner". "Donnie" is misspelled here. In all the shots after this one, there are sneakers, or some other equipment hanging over the "y's", hiding the spelling mistake. |
| 25 | I Love Lucy | In this episode Lucy is trying to get Ricky to notice that their anniversary is on Monday the 19th by showing him a large calendar with the 19th circled. However, when they are in California and they celebrate their anniversary and Ricky asks for information to be wired to him, the telegram tells him that their anniversary is on the seventh. |

| 27 | The Golden Girls | In earlier episodes we are told Blanche's middle name is "Marie", such as in the episode, "Wham, Bam, Thank You, Mammy" where in her ex-mammy calls her by her full maiden name (Blanche Marie Hollingsworth). But in this episode, she brings out her diaries to try to locate if she slept with Rose's husband Charlie, and the intials are B.E.D. - which stood for Blanche Elizabeth Devereaux. |
| 28 | CSI: Crime Scene Investigation | In the scene where Grissom is talking to the coroner about the man who was shot and strangled with his tie, you can see the actor that played the body breathing. |
| 29 | Sliders | According to reverse-time issue: In court, when Quinn looks at the clock on the wall, you can see that the clock still goes forward, despite of the anticlockwise indicators since the digits are inverted. |
| 30 | Lost | Ethan kills one of the survivors. Sawyer thinks it's Steve but Hurley corrects him and tells him that it was actually Scott. They make a point of Scott being the deceased and Steve being the one people mix him up with. According to the actors playing Scott and Steve it was in fact Steve who got killed but the producers messed it up. |
