Rank Title Mistake
iCarly Season 1, episode 10 "iWant a World Record": During the attempted record-breaking webcast, when Spencer is seen explaining his sculpture on the studio's screen the red leaf blower falls backwards, but when he is seen live the blower has fallen forward.
Dr. Quinn, Medicine Woman Just before Dorothy has her operation, Mike is looking through her medical books when Sully pays a visit of comfort. During their tender moment, you can hear slight foot shuffling from the camera crew in the foreground - which is not coming from the outside. Also, you hear a slight grunting sound during a slight pause in their conversation.
Bones Episode 4-18, "The Science in the Physicist": when Bones and Booth get trapped in the resonance chamber, the position of Booth's hands over his ears changes back and forth between shots. (Picture)
Buffy The Vampire Slayer When Buffy is talking to Riley, she puts the scarf on her head, on his hand. He talks to her and says "Maybe I am the bad guy," and in the next frame her scarf is back on her head again, then off again in the next frame.
SpongeBob SquarePants The broom that Patrick throws on the ground in Big Pink Loser disappears soon after.
Thunderbirds At the start of the episode when there are close-ups of Colonel Sweeney's face, studio lights can be seen in the reflection of his sunglasses.
Charmed When Piper and Leo confront the genie at the restaurant, the genie is eating food as fast as he can. At one point he gets a line of pasta sauce on his cheek. The shot changes, then goes right back to him only now the sauce is gone with no time for him to have wiped it off, especially since his hands are full of food.
House, M.D. When Foreman comes to inform the patients parents that the surgery to relieve inter-cranial pressure was successful, the mother asks "What happened? Did she hit her head at a meet or...?" The coach's audio responds with, "No, nothing like that," but the words don't correspond to his mouth movements.
Hannah Montana When Olivia turns up at Jackson's house, she says, "That was my Saturday afternoon. This is my Saturday night!" which indicates it is the same day. But when Cooper arrives, he says, "Sorry for blowing off at you yesterday," which means another day has passed. However, at the Hannah concert, it is the same day all the while, so Cooper had got his days wrong.
South Park Butters tries to get an "seamen sample", and is on it for two days straight. The mistake: You can see Butters' head in the school classroom, while he's still supposed to be in Cartman's bathroom.
Star Trek: Voyager If you look closely at the hot air balloon, in the shot before the message is attached to it, you can just see a wire holding it up.
How I Met Your Mother In the 7th episode of Season 5, the scenes when the "Storm-Trooper" (actually, a robot) passes by the window do not match. The first time we see it, from the outside, as soon as he passes, Robin notices him and points towards him, but the second time, from the inside, she doesn't point at him.
Glee In "Wheels," when Artie is singing 'Dancing By Myself' and doing tricks in his wheelchair, the wheels on the back of the chair that prevent him from falling backwards disappear and reappear, depending on whether or not he is performing a trick.
Sonic X Throughout the show, Dr. Robotnik is called "Dr. Eggman", even by himself and his robots. Dr. Eggman is the nickname the Sonic Team uses to mockingly refer to Dr. Robotnik. Dr. Robotnik hates to be called Eggman.
The Golden Girls Blanche and Rose are rehearsing their tap dancing routine. At the end of the routine, Sofia walks in and the boom mic becomes visible on the top right of the screen, moving toward her.
London's Burning Series 7 episode 7. In this episode Nick rides the Pump. However, being Station Officer, he should be riding the Pump Ladder. The London Fire Brigade always has the Station Officer in charge of the Pump Ladder and not the Pump (it's the Sub Officer who is in charge of it).
Castle In episode 2-5, "When the Bough Breaks," when Rick is talking to his mother regarding handing out her pamphlets at his book signing party, he opens his beer and throws the cap on the counter, when the shot changes you see him opening the beer again.
Quantum Leap When one of the sicko men from the van pulls a knife on Sam, the white blouse is either tucked into the skirt or hangs over it, depending on the shot.
Blackadder "Blackadder Back and Fourth": Blackadder and his guests are going down stairs. Blackadder is first, George is second, Melchett is third, Elizabeth is fourth and Darling is fifth. Blackadder gives his notebook to George. When they arrive downstairs, Melchett and Elizabeth are ahead of George. This is not logical, since the stairs are too narrow and they could not have passed George.
Seinfeld In this episode, after accepting a free Armani suit from Kenny Bania, Bania insists that Jerry repay him by taking him to a nice restaurant. Jerry takes Bania to Mendy's, a Glatt kosher NYC deli. During the ordering process Bania suggests, "Oh, get the swordfish. Best swordfish in the city. The best, Jerry." Swordfish is considered an unkosher fish by Glatt kosher standards since it does not have scales, one of the essential criteria. It would therefore never be served at Mendy's.
Malcolm in the Middle Towards the end, right after they watch the house blow up, you can see a hand holding a black object popping up from the right side of the screen. This happens right before Malcolm says, "Pretty sure".
Supernatural All the doors and windows get locked by the ghosts, trapping Sam and Dean inside, but they never even try to break a window or smash down the doors to get out, they just accept the situation.
Keeping Up Appearances At the beginning of the episode, when the TV repairman is at the door, you can see that the background behind him is a studio wall - not the front yard we normally see on that side of the house.
V (2009) During the opening scenes, as the Visitor ships are approaching, an F-16 is shown crashing. As the wreckage flashes by Morris Chestnut, the word NAVY can be seen on the wreckage. The problem is that the F-16 is exclusively used by the US Air Force. The Navy does not utilize any variation of the fighter.
Dexter Batista arrives at the office and after a short exchange with Deb and Quinn about his relationship with Detective Gianna, he greets Laguerta with "Mornin' Lu". The clock on the wall indicates it is 1:35 in the afternoon.
V After a talk with Diana on the mothership, Lydia is left alone and then she takes a message out of her uniform. She wears a ring on her left middle finger and her nails are painted red. In the following shot we see Lydia's hands put the message into a box. The hands do not have their nails painted and wear no ring whatsoever.
Two of a Kind At 2:55 in the kitchen scene when Ashley is baking cookies and Mary Kate walks in, a microphone is visible moving across above their heads.

Auf Wiedersehen, Pet Dennis and Neville are push-starting Oz's car after they have stopped for a break. This wouldn't help, as the car has an automatic gearbox. The automatic gear selector on these cars (Ford Zephyr) is on the steering column and can be seen when they break down on the motorway.
Highlander Tessa finishes Duncan's sentence saying that the last time he wore a tuxedo was on the deck of the Titanic, however, in a previous episode he's shown wearing one at a New Year's Eve party in the present day. After fighting another Immortal, he puts on the jacket to hide the bloodstain on his shoulder before returning to the party just in time for the countdown.
