| 1 | ER | When Morris is on the bed following his shock there are three point of view (POV) shots. Morris on the bed, Morris's POV of the medical staff hovering over him and a shot from behind the staff looking down on Morris. The placement of characters is misaligned between views, most notably when the shot is from behind the staff looking down: there is a sizable gap between the two on the left and right, where as from Morris's POV they are clustered close together with no gaps between the staff members. |
| 2 | Dark Shadows | 1897: Quentin challenges Barnabas by pulling a decorative sword from a wall display and pointing it at his throat. Unfortunately, the display wasn't all that well attached to the set wall. Though the camera pans away, you can hear a loud <i>clunk</i> as the piece falls, followed by the loud crash of shattering glass as it strikes and breaks a table lamp. You can also see the actors struggling very hard to keep straight faces as they continue with their lines. |
| 3 | The Simpsons | During the family's breakfast, in most shots Marge sits right beside Maggie's highchair, but in certain close-ups they are quite a few feet away from one another. |
| 4 | Red Dwarf | When Lister says "he was in sickbay for a week", once the shot changes we once again hear him say "for a week". Evidently, the line was intended to carry on into the next shot but wasn't edited out. |

| 7 | Without A Trace | When Martin recognizes the bully in the arcade, this kid just passes two pinball machines. When Martin goes after the fleeing kid, he passes just one pinball machine. The left machine has disappeared. |
| 8 | The West Wing | Josh is rushed to GW hospital with a collapsed lung and ruptured pulmonary artery. However, when Toby finds him at the shooting scene, Josh is holding his left abdomen, and the bullet, being fired from the building across the street, would have ranged downward, far from the pulmonary artery. |
| 9 | Doctor Who (2005) | When the Doctor is proving his identity to Harriet Jones by telling her about when they were trapped together at Downing Street, his words don't match the movements of his jaw when we see him from behind - the timing's all off. |
| 10 | Supernatural | Near the end of the episode when Dean is being chased by the possessed truck, Sam tells him to take a left. After he's made the left, Sam tells him to go exactly 7/10ths of a mile and stop. Dean looks at his gage and it reads 70098.3. Dean looks up, then back at it: it changes to 70098.5, but the next time he looks down it says 70100.6 and when he stops it says 70101.0, meaning he's gone 2 miles too far. (Probably meaning it took two shots to get the scene right but they messed up on the gage.) |
| 11 | Lost in Space | When the Robot is returning to his beloved with the energizer, he gets near the right side of the shot and abruptly vanishes rather than rolling off camera normally. |
| 12 | Time Trax | "Treasure of the Ages": In the beginning, one diver stabs the other, releasing a cloud of blood into the water. In the very next shot, however, not a trace of the blood remains. It couldn't possibly dissipate in that short a time. |
| 13 | The Big Bang Theory | The Loobenfeld Decay: When Sheldon is knocking on Leonard's door at the start, the sound is out of sync with the action. |
| 14 | Stargate: Atlantis | In the scene where the Wraith attack on the Mainland, Sheppard is stunned and falls with his right hand on his gun near his hip, but when McKay and the other guy grab him, to pull him to the gate, both the gun and his right arm are above his head. |
| 15 | The Green Hornet | "Ace in the Hole": When Reid is showing the DA his closed-circuit video of Mike, several pieces of white tape stuck to the carpet are visible. These are blocking marks, put there to show the actors where to stand. |
| 16 | South Park | When the kids are shown in Token's backyard, Kevin is not present. But a few seconds later, when they are each giving their opinions about the 'cursed' video, Kevin suddenly appears on the end of the line. |
| 17 | Battlestar Galactica | Early in the episode a damaged Viper makes its way back to Galactica with its port-side gun trailing smoke as if in an atmosphere. But in a vacuum, the smoke wouldn't trail. It would build upon itself much the way shaving foam does when slowly leaking from a damaged can, unless the ship changed course and/or speed and then the smoke would appear to do the opposite - ship turns right, smoke moves off to the left because it remains on its original course regardless of what the ship does. The gun is not propelling the smoke rearward. The ammo inside is simply burning. |
| 18 | Star Wars: Clone Wars | After his duel against Dooku, Grievous is seen putting his saber away. When he puts it on the belt, it has no spikes, but in the close up two spikes can be seen at the bottom. |
| 19 | 24 | At 18:50, Tony tells Jack that he's contacted someone at the San Jose Sheriff's Department. At 19:35 Jack calls and gets a response of "Sheriff's Department" on the line. There is no San Jose Sheriff's Department. It should have either been the San Jose Police Department or the Santa Clara County Sheriff's Office. |
| 20 | Friends | When Ross tells Rachel he can't see her anymore, the door to Monica's room is open revealing a completely blank and empty room. |
| 21 | Star Trek | Spock presents himself to Dr. McCoy in sickbay to be relieved of duty for "killing" Captain Kirk. (It's done, of course, so Kirk can walk out of the room and surprise Spock by being alive) But Bones has no command authority since this is a legal/military matter. If Spock believes he should be relieved for breaking regulations and committing murder, and thus confined until court-martial, Scotty would be the one to do it, as he is the next senior officer. And this is not a character mistake, Spock quotes regulations regularly, there is no way he'd ignore chain of command. |

| 24 | CSI: NY | Episode 19 - Personal Foul; Lindsay is going through the cheerleaders' photos to find a biometric match of the heavier woman, and she picks a head shot of a blond cheerleader. She removes it from the pile to scan it, seeing through the back, we can see what the actual picture looks like. It is a close up picture of a woman wearing big sunglasses. When the scan is complete, it is revealed as the original cheerleader photo. |
| 25 | Gilmore Girls | During movie night, Dean mentions that he has sisters. But in subsequent episodes, he only has one sister. |
| 26 | Heroes | When Peter and Claude are invisible and walking in New York, just after Claude has released his hold on the T-shirt rack, they bump into a man in a coat. Cut to a different angle, showing the two on camera, and the man in the coat is gone. Instead there are two girls and a man with glasses walking directly behind Peter and Claude, none of whom were there a split second ago. |

| 29 | Monk | In the scene where Darwin and Monk are in the kitchen and Monk calls Dr.Kroger. Darwin pulls the phone out of the wall, but there is no line from the phone to the jack in the wall. |
| 30 | Scrubs | Throughout the series, JD and Turk are shown to be major sitcom geeks. At one point in the episode, JD says to Turk he Marcia Brady'd his ass, and explains what it means. Turk responds along the lines of "Yea, I know. 'Marcia Gets Creamed' Season 5 episode 3. Don't ever question me about The Bunch!" 'Marcia Gets Creamed' is season 5 episode 10. Episode 3 is 'The Elopement'. Not a mistake a couple of geeks would make. |
