Visible crew/equipment: You can see a crew member hand Janet a stack of papers behind her back.
Chapter Two: The Weirdo on Maple Street - S1-E2
Factual error: When Chief Hopper and his fellow patrolman are standing over the lake, Hopper has a radio on his belt. The radio is a modern Motorola walkie talkie and at least a decade newer than it should be.
Continuity mistake: Farzad JinJi is introduced as Dominque in the first season, but he appeared as a different character in the second episode.
Character mistake: In the 11/14/2005 US Army Rangers report about Peter MacLeish, the word captain right under MacLeish's signature is misspelled "captian."
Chapter One: The River's Edge - S1-E1
Revealing mistake: When Hermione opens the bag of money, one stack says "Motion Picture Use Only." (00:29:45)
Continuity mistake: During the photo shoot, after Tony takes the final shot and tells Margaret to "get dressed" so he can show her his place, she is wearing a different blouse when she comes upstairs to his apartment.
Factual error: When Riggs and Murtaugh go to investigate the scene where the body of Ramon Alvarez is discovered Scorsese states that Alvarez's dog tags say "SEAL" on them. His dog tags would not say that he is a SEAL on them. The information listed on dog tags is as follows: Name, Social Security Number, Blood Type, Service Branch, and Religious Preference. There is no way to tell what a given service member's MOS (job) from his or her dog tags. (00:14:00)
Continuity mistake: When James is taking pictures from a car as men are entering a hotel, in the shots of him head on he is using a Canon EOS camera with a 70-200mm white lens. In shots from the side he is using a Nikon camera with an older Nikon lens. (00:27:35)
Factual error: The show starts in 1901 with a woman constable working for the Metropolitan Police Service (Scotland Yard). However women didn't join the Metropolitan Police until 1919 (which is also when the first woman's police uniform was created). Prior to that, women volunteered in the Women's Police Service, but that wasn't formed until 1914.
Plot hole: They only take out the shrapnel from the gut shot - they never deal with the shrapnel in his shoulder at all. (00:04:25 - 00:05:00)
An Unwelcome Distraction - S1-E4
Continuity mistake: When Dwayne accidentally fires the gun, we see the slide lock back in place automatically, meaning it's out of ammo. In the next shot the slide is back in position, and we don't hear him releasing it.
Seeds + Permafrost + Feather - S3-E17
Factual error: Bozer and for that matter I would assume all of the team went to a spycraft academy. A basic subject in any of such academies is surveillance, including proper techniques to follow and stake out someone. Bozer being so inept at tracking MacGyver's father is inconceivable given their training.
Factual error: Captain Souter is shown lying dead in the snow following the massacre at Gandamak. In fact, Thomas Souter was one of the few who survived to be captured by the Afghans and was later freed.
Other mistake: The Lakota is attacking a small group of white men. One is face down on the ground, and he lifts his head to cut his throat. In the background are two cowboy actors joking and twirling their guns. One drops their gun and picks it back up. (00:20:43)
Episode #2.3 - S2-E3
Continuity mistake: When Belinda and Fleabag are having a drink, Fleabag's hairstyle keeps changing in every shot: sometimes she has a bang of hair falling over her forehead, others not, others it's loose, others it's protruding slightly.
Continuity mistake: During the Medici brothers vs. Marco Bello confrontation near the end of the episode Lorenzo grabs a candlestick to defend himself against Marco Bello's knife - you can even notice a candle slipping off the candlestick. As Marco Bello turns to leave the chamber, however, Lorenzo's hands are empty and the candlestick rests on the table. In the next shot he's holding it again, with the candle sitting atop of the candlestick. (00:47:30)
Continuity mistake: After the dance, when Jack is talking to The Big Three, he grabs the towel and puts it on the other side of Rebecca. When the scene cuts back to Jack rubbing Rebecca's belly, the towel is still on Jack. Then, when Rebecca's water breaks, it is back to being on the other side of her. (00:03:40 - 00:04:12)
Other mistake: When Cobb starts to chase the red Dodge, there's a shot of him passing between two vehicles (and very distinct buildings in the background). When Murphy picks up the chase, this same shot is reused, showing her passing between the same two vehicles.
Factual error: "Underground" is set in 1857 Georgia, yet the padded baseball glove August Pullman gives to his son is of a type used during the early 20th century. Before 1883 when padded gloves were first manufactured, gloves were either not used by baseball players or railroad brakeman's gloves were used.
Suggested correction: Actually no, in that angle and the way Jody kicked him away hard from the truck it's possible he landed a lot further away than you might think.

Before I submitted the mistake, I did take into account the angle and the distance that Jody kicked Bruneau, replayed the scene several times, and he did disappear, as there was a sidewalk and a length of lawn leading up to the apartment building where Bruneau should have been lying as Jesse and Jody took off in the truck.
